Ask A Doctor

I had a hysterectomy due to abnormal cervical and uterine cells. But the pathology report showed that I in fact had stage 1 endometrial cancer. I’m now well into my second round of worrying and fretting as I prepare to see a gynecologic oncologist. What now?

Some women do find out that they have gynecologic cancer as an unpleasant surprise following a hysterectomy since tissues removed during a hysterectomy are routinely examined for abnormalities. The symptoms of some types of GYN cancer can blend into other gynecologic and pelvic problems, and the treatment of one sometimes reveals the other. The good news is that it was caught very early, and your prognosis is therefore excellent.

The next step is to go see a gynecologic oncology specialist, which you are already preparing to do. A gyn-onc specialist will be able to properly set a grade and stage for your cancer, which will help determine if further treatment is necessary, and if so, what kind. Assuming your hysterectomy was done by a gynecologist who didn’t suspect cancer at the time, he or she probably did not remove any lymph nodes. Your gyn-onc may want to remove or sample some lymph nodes in your pelvic region to see if the cancer has spread anywhere else. If so, you will be treated accordingly to kill the remaining cancer. If not, and the hysterectomy seems to have removed all the cancer, you will probably be advised to return every three months for monitoring.

As always, we recommend getting a second opinion whenever possible. Some gynecologists and gyn-oncologists are more aggressive than others in the way they choose to treat cancer. Ask lots of questions; bring someone with you to appointments to help you remember what is said; and don’t be afraid to ask another doctor for his or her take on your treatment options.
