I have a planned hysterectomy with bilateral salpingo oophorectomy due to complex endometrial hyperplasia with atypia. I am 49. Removal of ovaries is standard treatment, but I am wondering if true risk of cancer is less than risk of heart disease/bone loss from loss of ovaries/estrogen & other hormones.

1. CANCER. No further explanation needed for this.

2. The average age of menopause (defined as no periods for a year) is 51 years. That means it is around the corner for you if you keep your ovaries. After menopause the ovaries do secrete small amounts of hormones, and there is a little information that preserving them may add a few months (only) to your life. Around and after menopause you will lose estrogen with or without the ovaries and will need to be doing the usual things to preserve bone mass with or without them.

3. There is no evidence that retaining ovaries after menopause improves the risk of heart attack or stroke.

4. If you are overweight or obese this would be your number one risk factor for a heart attack or stroke, along with diabetes, hypertension, joint problems, and several types of cancer including endometrial (which you almost have), colon and breast cancer. Obesity can take 20 YEARS off your life. Any benefit from retaining ovaries would be lost due to this HUGE health risk.

5. If you do not have cancer found after your surgery you could be a candidate for estrogen replacement therapy which will help with bone loss and is not a serious risk for breast cancer (the risk is mostly if you need progesterone to go with the estrogen as women who have a uterus do).
