My doctor said I have the option of either having a complete vaginal hysterectomy or a laparoscopically assisted vaginal hysterectomy. Which one is the better option for making sure that all signs and such of pre-cancer or cancer are removed this first time? I do not want anything to be missed resulting in further surgery or treatments such as chemo or radiation.


With a complete vaginal hysterectomy, it can be difficult for the surgeon to clearly and carefully explore the pelvic and abdominal regions for all signs of cancer. Additionally, visualization is limited to what can be seen by the naked eye.

During a laparoscopically assisted surgery, the laparoscope allows for magnified visualization as well as room for the doctor to more thoroughly explore the pelvic and abdominal regions for any signs of problems.

Another surgery option to consider is one utilizing the daVinci robot. This can allow for ten times the magnification as well as the room needed to look for problematic areas. Furthermore, there can be greater precision abilities with the robotic tools.

You should also consider that with a cancer concern, a frozen section pathology or cold knife biopsy may need to be done during surgery to help determine if cancer is present. This can be important as if there are definite cancer cells, it can be best to have your uterus and any tumors removed whole rather than cut into pieces. This would prevent stray cells from being left during surgery resulting in more surgery and/or further treatments.

Before determining which surgery is best for, it would be wise to get at least a second opinion. With a cancer concern, a consultation with a gynecological oncologist would be a wise choice. You may also want to find a surgeon who specializes in using the daVinci robot to see if that option might be the right for you.
