Ask A Doctor
Can you explain the stages of endometriosis to me?
Most doctors recognize four stages of endometriosis, though staging can be subjective depending on the chart and criteria used.
Staging can only occur with surgery, which is also how endometriosis is diagnosed. Staging is not necessarily determine by the amount of endometriosis, but more by the size and depth of the implants and the severity of the adhesions.
The four stages are generally determined as follows:
Stage I refers to minimal endometriosis, exhibited by only a few superficial implants.
Stage II refers to mild endometriosis, exhibited by more and slightly deeper implants.
Stage III is considered moderate endometriosis, exhibited by deep implants, small endometriomas, and some filmy adhesions.
Stage IV refers to severe endometriosis, exhibited by many deep implants, large endometriomas, and dense adhesions.
The one thing to remember: the stage of endometriosis does not dictate symptoms. Women with very minimal endometriosis can experience excruciating pain while those with stage IV endometriosis can be asymptomatic. For this reason, not all doctors even stage the endometriosis of their patients, instead they treat based on the symptoms the individual patient is experiencing.
