ovarian cyst and pelvic pain
Ok Ladies, I am a 44 yr old nurse but not an OB/GYN nurse and am scared to death about the prospect of surgery but cann't live like this.
My new OB/GYN perfomed a vaginal US in March due to irregular periods and horrific pelvic pain. Found Right ovarian cysts but didn't seemed overly impressed but gave me the options of living with it, pain meds or a total hys. Thought well, lets give this a month or two or three and see what happens. WELL, since then diagnosed with Insulin Resistence Syndrome( Yes goes with POCS) and probably the ovarian cysts WONT go away on their own.
Now with very irregular periods, three to five days of horrible pelvic day(reminds me of labor after 16 yrs.) and unbelievable emotions and bleeding.
My question is,I have an appt next month with the OB/GYN, what do I ask him? I appreciate any advice in this area. Thank you

This is the same thing that I'm going through. I have been like this for the last 6 years. I thought it would get better, but it hasen't. If you dont want a Hyst yet ask for Progeterone, or some other type of birth control. I have tried this, but I'm allergic to the progesterone it causes me terrible side affects. Hope this helps

I would love to try BCP or anything but do to my diabetes and the dirty habit called smoking(and No I cann't quit, I would truly be unable to live with) I cann't due hormones.
I am hoping my endocrinologist can find me some relief. I just got off the phone with my OB/GYn have appt next month and he is thinking a total hys. My husband says go for it... but its not his uterus and ovaries their taking... That would be fun to watch.
I love this is site..its nice to know I am not alone.

There is also a wonderful site for PCOS ladies called Soulcysters.com where you can also get further information specific to that condition. I do know that there are some specific medications that help...and hopefully you're working with a good endocrinologist on the metabolic syndrome issue as well.
Good luck with the smoking...it's not easy, as my DH knows all too well. But since metabolic syndrome also goes with hypercholesterol, etc., can your endocrinologist or internist try Zyban? It seems to have helped a number of people that I know.
You do get rid of the cysts with this surgery...but discuss the hormone implications of surgical menopause with your endocrinologist. That may make some difference in your decision-making process!
Good luck with this...I know it's a hard decision to make. Do a search here, also, under "PCOS" ...there's a lot of fellow sisters out there!

Hi Mel-
Just wanted to say It sounds like you have some of the same medical issues as me. I had a TAH/BSO in June and am very happy with my choice. I am 39 and I am on HRT, estrogen only. I feel great and also have had less migraines since getting the patch sorted out. I can't believe how much better I feel without all the pain from cysts, periods, etc. I was glad to get rid of those pesky ovaries because they never worked right and only caused me pain!

Hey just saw my endo dco and just diagnosed with insulin resistance syndrome....GREAT. Now have triglycerides of 281 and cholestrol of 210. However at least my insulin level and fasting glucose are ok.
Started on Topamax 3 weeks ago and he thinks this is making my periods weird and painful due to my uterus sloughing...OK... then he proceeds to tell me when I mention how bad the pain is that the pain must be bearable or as he put it "I am still here"... and to hang in there it will get better. Also not to go for a hyst because it isnt the problem, the insulin resistance syndrome is causing ALL of it. I had to be very blunt with him and remind him he is a man and doesnt have a CLUE!!! I think its time for a new endo doc. The last thing I need is a male telling me how I feel.
What do you all think?? Wait this out or go with my OB's opinion ?

This sounds like full-blown metabolic syndrome, you poor baby! There is, however, good news, as with the proper treatment, you should be able to get that down to some better level. It's do-able, but does require some discipline and the right combination of anti-lipid therapy. Luckily, these days, there are more choices than ever. Also, if you're able to quit smoking, that will improve your profile as well.
Have you asked your GYN to actually call up your endo and speak with him? He may not realize the magnitude of your GYN woes, nor understand what the appropriate therapy may be. It's a shame when someone who has a condition as complex as metabolic syndrome has to wind up with such fragmented care...but then that's the reality of today's "medicine"
Take a look at the website that I linked for you previously...among other offerings (and it's a very professional site, lots to offer) is a nationwide list of doctors who specialize in PCOS issues. Maybe it's time for a second opinion with a GYN who specializes in this overall metabolic problem.
I know this is difficult (not firsthand, but have friends who've struggled with it). But I hear that the changes CAN be made, and at least improve the metabolic profile. It's good news that your fasting glucose and insulin levels are OK...have they also done post-eating levels? And have they looked at the hemoglobin A1C? As long as that's steady, you're in good shape in the glycemic front, at least. There is a lot of information on this site as well about glycemic index, etc. that may help.
A suggestion in the dietary department: there was a study published recently (it may have even been an arm of the Nurses' study, I'm not sure) that looked at the effect of peanut butter (NOT Jif, of course, but "real" peanut butter) in diabetic women. It had an interesting effect: it improved their blood sugar control...and apparently it had some sort of cholesterol impact as well, although I don't recall the details. It would be worth searching for on the internet...as I recall, the results were pretty amazing.
Please, do ask your GYN (or whichever of the doctors you trust more, but right now, that sounds like your new GYN) to call your endocrinologist. And, if you feel that the GYN also isn't really listening to your needs, take a look on that list and see if there isn't someone near you to see who specializes in PCOS and can help you sort out some direction for yourself.
