Is Biest tough to prescribe?
I have had 3 different doctors now tell me they are wary of Biest because it is not easy to figure out how to prescribe it. The doctor I saw today also felt that with the creams, it is harder to be sure you are getting a consistant dosage. It seems like on this board a lot of people use it and have good luck with it. So what is it? Do you have to first decide what ratio of estradiol to estriol you want and then figure out the dosage?
Anyone know what is a good place to start and how to tweek from there? If it's not right do you adjust the dosage first and then play with the ratio. I think my new doctor seems open to it but it's not her first choice.

I'd recently switche to Bi-est from a .075 Climara patch. My doc wanted to match the dose of a .1 mg Climara patch so she prescribed 1.25 mg 80:20 Biest 1X per day. My pharmacist said the 1.25mg 80:20 was a weak prescription for Bi-est.
The results for me? I started showing the same symptoms of low Estrogen that I hadn't had since just after surgery almost 2 years ago. So after several months, I switched back to the .1 mg Climara patch. That first day back on the patch I was getting such a rush of Estrogen that I was feeling dingy. So for me the 1.25mg Bi-est performed way lower than the .1mg Climara patch.
Interesting.

When I was switched from the .1mg Climara, I was given 2.5 mg bi-est. It seemed to be a pretty even trade.
I found my Dr by asking the compounding pharmacist for a reference. My Dr and my pharmacist work together on the dosing.

Your pharmacist can be a big help if your doctor isn't sure how to write the script. I am using 2.5mg bi est, which is 2.0 estriol and .5 estradiol. I've been using either tri est or bi est for over 7 years now. It's worked out well for me.

many thanks. At least my new doctor seems willing to work with me and I have found a compounding pharmacist who deals with hormones. Kay M. Do you feel different on the biest than you did on Climara?

I was pretty swollen on the Climara, but that could have been a dosing issue. The main reason I was changed was because I am extremely insulin resistant and estradiol can increase insulin resistance. Estriol doens't appear to have that effect.
I'm glad you have a new Dr who is willing to work to find what is the best HRT for you and your health issues. Remember to be patient as it can take several weeks (up to 5-6) for your body to respond fully to any change. It can take a while, but each "tweak" moves you a little closer to the balance you are looking for.
