Not sure what to do ???

Hi everyone, I'm 50 yrs old. Have been having lower back pain , abdominal pain, incontinence for some time. Finally went to gyn when just couldn't stand it anymore. I've got a 10 cm ovarian cyst on the right that my gyn says has displaced the uterus and is pushing on the bladder. Also have a smaller one on the left. CT scans and tumor markers came back non-cancerous. Gyn says both ovaries need to come out and recommends that I have a subtotal hysterectomy (leaving the cervix in place). He did mention that he might not be able to do it laparoscopically and then would have to go vaginally (I'm assuming then the cervix will have to come out). I do have an appointment with him tomorrow. Basically I'm wondering maybe I don't need the uterus removed since I have no fibroid, but then again I sufferred horrible heaving bleeding until having a IUD put in with they took out almost 3 weeks ago. I'm concerned about losing the cervix and all the the uterus and some of the possible negative outcomes (loss sex drive, prolapse, incontinence) but the other half of me says "just take it all out". I'm so confused !!! I do know I want this done as soon as possible and living in small town a second gyn opinion will only postpone surgery.
Here is the exact write out on the preop form : (written by the gyn):

"laparoscopic suprocervical hysterectomy with bilateral salpingo-oophrectomy, possible laparscopic assisted vaginal hysterectomy possible laparotomy"

There are sure a lot of "possibles" . Also what is "possible laparotomy" at the end ?

Also, my Gp says that it might be hard for him to do the procedure laparoscopically due to that I'm a larger women (220 pds/ 5'4) and if he can't then he would do it vaginally---then I assume I lose the cervix ???????

Re: Not sure what to do ???

My other option could be take both ovaries but leave the uterus. Then again my mom had her hysterectomy at age 49 due to very large mass in her uterus (along with some on the ovaries) (she doesn't remember to much about what the actual mass was). So also wondering with her history if maybe a total hysterectomy is the better option

The choices can be very confusing! That is one reason why we suggest all women seek a second opinion.

If you do have a hysterectomy and it does turn into a vaginal or vaginal assisted surgery, yes you will lose your cervix. And the "possible laparotomy" means that if the surgery cannot be done through smaller laparoscopic incisions you may need a larger abdominal cut.

I can't tell you which options are right for you. I do recommend the least invasive options first and that you not have a hysterectomy unless you are 100% sure it is the right choice for you. I also strongly recommend that you seek a second opinion before making your final decision. A hysterectomy is not a risk free surgery and one should be very sure it is the right option for her and that she has the best surgeon to perform it before heading into OR.

I hope your appointment goes well tomorrow!

Re: Not sure what to do ???

Thank you for your reponse. I postponed my appt to give me more time for research . What Im thinking is if they can't get to ovaries with a scope and vaginally I would loose the cervix (which I want to keep) and uterus (not sure on that one) then my only other option would be the abd procedure which equals a much longer recovery
