TVH Blood Clot 3 week post op?

I had a TVH on Oct. 4th. I started bleeding this past Sat. which was almost 3 weeks post op. Yesterday, my dr. told me the stitches may be dissoving and I have only lost one stitch from the vaginal cuff. More bleeding than ever last night and then it slowed down until this evening. I've been using nothing more than panty liners because most of the blood comes out after I urinate; I guess from pushing. An hour ago, when I went to wipe after urinating, I noticed more blood on the tissue and a two inch long thick blood clot. Should I call the doctor again??? I feel like I am being bothersome. Why would I be passing blood clots? Could it be from the exam yesterday? He had to rinse the inside a little because of the blood build up and then he swabbed it. Any advice?

It could possibly be from the internal exam you had yesterday and leftover blood that he missed.I was told by the OR nurse that I could expect clot or two when the stitches in my cuff disolved
