Anyone have thoughts on TVH speading dysplasia to vagina?

I saw on another web site(don't remember which one) that if you are having hyster due to dysplasia that they should do abdominal hyster so when they take out cervix they don't spread dysplasia to vagina. Anyone have any ideas or feedback on this "sort of question".

Hi!

I don't have any experience with this, but I am hoping another (((sister))) will come along who can shed some light on the subject for you.

For what it's worth, a Google search of "vaginal hysterectomy for dysplasia did turn up a few articles... but you'd have to read them more carefully to see if they actually give a conclusion.

Interesting question.. had severe dyplasia a few years back but never thought about this. Mine was treated w/ a LEEP and have been ok since so guess that wouldn't be much of a concern now for me but I can see what you're asking. I would point out the article to your dr and get his opinon. Hugs!

The only "2 cents" I can provide is that I had a LEEP 9 years ago. Had an ablation Sep 06 and TAH 10 Apr for Adenomyosis and adhesion. No mention of spreading dyplasion as the reason for the TAH -- more concern about the uterus size & clean up.

However, I am scheduled for an annual in Sept of this year. Asked "why" and I was told that cervical cells can still embed in vaginal walls post sugery and that a PAP is still necessary.

df323-this is my second time with dyspalsia, had laser durgery in 2003 (CIN III) now it's back at CIN II and NOT showing up on pap test, only showed up on biopsy.
teri_j -still planning on get paps, I have a appointment on June 11 for second opinion before I have ANYTHING done. Thanks.

Hi strongredhead,
I looked upt the surgical procedures for cervical cancer in the May Clinic Guide to Women's Cancers and in ther it says that the surgery is either an abdominal incision or through a vaginal hysterectomy. It seems that the stage is a factor in determining which procedure. They may be recommending the vaginal procedure if it is a precancer.

The advantage of the abdomiinal is that while they are in there, they can have a better visual of other organs to make sure all is good. Are you seeing a gyn/onc? If not, you might want to consult one as they are the experts in gyn cancers and precancers. Ooops, I see you are in Canada so am not sure what the procedure is there for seeing a specialist.

I have not read any medical research indicating that a TVH increases the risk for dysplasia spreading to the vaginal wall. Actually, it's quite rare for dysplasia to recur on the vaginal wall, though it does happen. And it's quite possible that the recurrence on the vaginal wall for many women is really just cells that were not removed during the initial surgery (not a wide enough margin was removed) that have now multiplied and spread.

My surgery was TAH because I'd had so many previous abdominal surgeries and it was assumed that there would be way too many adhesions to do it vaginally.

Thanks Jeanine- How do you know if they remove a wide enough margin? I guess you don't until it shows up somewhere else later. I'm going to ask GYN #2 at appointment #2 and see what she thinks my best course of "action" is. It's freaking me out that dysplasia ISN'T showing up on paps, ONLY showing up on biopsies.
