How big of uterus is too big for vaginal hyster
Hello,
I feel like I always have questions. at my appointment on Friday, my doctor said my uterus is probably too big to do a vaginal hyster. He said I will probably have the old fashioned abdominal.
He said my uterus is 8-10 weeks, fibroids, and possible endo....which hasn't been recently diagnosed (last diagnosis was almost 20 years). All in all since I am refusing Lupron, he said I probably won't have the surgery the way I had hoped.
Anyone have a uterus this big have a vaginal instead of abdominal hyster? Thank you!

I was told that by my previous gyno before I found this one who does laparoscopic. She said she would know more once in there but for now Im hoping for the vaginal laparoscopic surgery.

From what I understand it also depends upon your doctor. Some are more proficient at the laparoscopic than others. You could get a second opinion to see if another doctor would be able to do it the way you want. Or you could find out that your first doctor is right and it is too big.

Based on what you are sharing, I strongly recommend you get a second opinion as you and your surgeon are not on the same page. You need to have full confidence with your surgeon before heading into OR, and with an endometriosis history you need a surgeon with significant knowledge, experience, and skill treating that condition.
When determining when a uterus is too big for a particular surgery type, there are several variables involved, including the skill of your surgeon, the diagnosis, and your body structure. That said, I don't advise a straight vaginal hysterectomy for anyone with an endometriosis diagnosis. It does not give the surgeon room or visual to find and remove any existing implants. Since a hysterectomy is not a cure for endometriosis, it is important to have a surgery type that will allow for any of the existing implants to be found and removed. It is also important that the surgeon be someone with exceptional skills when it comes to endometriosis. That means it is not likley to be a general OB/GYN.

Mine was the size of a woman who was 5 months pregnant said my OBGYN. She said she has never seen one that big. Took it out laproscopically about 3 yrs ago. No problems so far except some menopause symptoms. Taking progesterone for that. I'd get another opinion, cant hurt.

I was told mine was too high for a vaginal procedure and happens to be back to front as well! The other component of the decision to go with an abdominal was that the doctor wanted it out in one piece to get clear margins for my cancer

Mine was the size of a 20 week pregnancy and it was taken out vaginally (in several pieces) during my TLH. Like the others have mentioned, there are a number of factors, but it's best to get a second opinion.

My uterus was the size of a small playground ball, too big for vaginal so she removed it laprascopicly by cutting it into small pieces. The woman is skilled, it was a long (4 1/2 hours) surgery but it has made recovery that much easier. But it also depends on your doc's experience and training and the equipment at your hospital.

My uterus was very elevated, fibroids everywhere, 7month pregnant size. Both my gyn and oncology gyn said TAH, vertical incision. My onc gyn has done laproscopic for large fibroids, but when he examined me, he knew right away it would be TAH. I have been having a very good recovery, and also glad I relented to vertical incision, because that meant less muscles to be cut. I also refused to take lupron, as suggested by two gyn's, and glad for my decision.
