Total/ sub total hysterectomy?
Hi everyone,
I am due to have a sub total hysterectomy in January, with the option of making it total, to take away my ovaries and cervix. I am 50 years old and not menopausal yet. my womb has a 4ins fibroid and lots of little ones, I have been offered embolisation, cutting them out or the hysterectomy, the first 2 options will not get rid of them completely and the latter will. I have had painful periods for a while and every month it feels like my stomach is being scraped from the inside. plus I always seem to flood and make a mess of my sheets. I have a weak bladder, which my gynae feels is aggravated by the fibroids, When I have been to the toilet I always end up wet after I have dried myself.
I am writing this on here as I would like your advice please, my mother died of Ovarian cancer, but because she was in her 70's I should not be at risk, also I have heard about being at a higher risk of strokes and heart attacks if they are removed, also does removing the cervix take away the enjoyment in making love?, also I was told that by removing the womb your bladder has nothing to stick to, which can lead to prolapse, I am concerned that I may become incontinent?javascript:emoticon('Embarassed')
I appreciate you taking the time to read this and given any advice that you feel might be helpful. I guess it is fear of the unknown. Thanks.

Re: Total/sub total hysterectomy?
Hi Healthyandwise!
It's good you're asking questions! I don't think there is any scientifically-proven advantage in keeping the cervix, but it's up to you (with advice from your doctor, of course) to make the decision that's best for you.
I opted to have my ovaries removed at the same time as my hysterectomy, since the doctor would "be in there anyway" and because cancers of many kinds -- although not ovarian cancer, so far -- run in my family. I do have to take HRT (hormone replacement therapy) to replace the hormones my ovaries used to make but the cost in doing so is manageable for me and taking one pill a day is not a big issue.
I hope that in partnership with your medical provider you make the surgical decisions that wind up being the best ones for you!

Re: Total/ sub total hysterectomy?
Regarding your ovaries: Have you talked to your doctor about the risk of ovarian cancer? Have you had a CA-125? My doctor and I discussed various options based on what she found when she got "in there." You and your doctor might agree to something like leaving your ovaries if they look good but taking them if anything seems fishy.
How was menopause for women in your family? My mom had a terrible time starting in her early 40s. She took lots of HRT back in the days when women with ovaries took it regularly. I started getting night sweats 48 hours after my surgery. I think my mom and I are wired to need that estrogen.
I can look at this two ways: I've made the problem worse by getting my ovaries removed or that I'm on the same path as my mom but things will be more consistent and predictable for me. So, I would think about how differently it might be for you compared to other women in your family.
Bladder problems were my first concern. I had ovarian cysts but my uterus was fine. She said that most incontinence comes from the angle of the urethra and not the bladder itself. Also, there are surgeries to fix that. That's just her opinion so I would ask your doctor.
Best wishes and a speedy recovery,
Cynthia
