confusion after post hysterectomy appointment.

So I was able to look at some of my documents online through my hospital (my health portal). On them it stated that I had hypertrophy of uterus and endometriosis. Non of these showed up on the ultra sound that I had (abdominal and trans )
The lab report of my uterus said that they didnt see adenamosis (spelled wrong on my end sorry) Then in the bottom of the report it said there was a focal adenaymosis. It didnt state endometriosis on the report it also said that I had only a small amount of enlarge uterus.
So this all being after I was loosing 400ml on my heavy day of my period for the last year and a half.
At my doctors appointment i asked him if there was endometriosis inside my body that they had to clean out. He said no. He stated there wasn't any. I asked why it was put on the report. He said that the adenaymosis was a form of endometriosis. I didnt think that was true??? Now I am just really confused. I wonder if they just wrote something down as a diagnosis of the heavy bleeding for insurance purposes. Any thoughts?
Also I am still having odd pain when stool moves through my bowels...random areas ( i think under where the incisions were and possibly near where vaginal cuff is) Did any one else have this two weeks post op?

The surgical report usually has an initial diagnosis prior to surgery, while the pathology report is after lab work was done and I feel it's more reliable. I think your doctor confirming that no endometriosis was found or cleaned out is a good indicator that there wasn't any endo. As for him saying adenomyosis is a form of endo, I think he's giving you a simplistic explanation. That was once believed to be true, but more research has separated the two. What's on the portal could be somebody's error in keying things in, or confusing it with something they keyed in right before. If it bothers you, you could ask your doctor if it could be corrected. You've seen the actual documents and talked to your doctor, and that's what counts.
