Confused about colposcopy results

I am scheduled for Da Vinci hyst in a month. Have had 2 LEEPS in 2 years, with followup paps coming back CINIII. LEEP in Sept came back CIN II. Dr did colposcopy last week to rule out micro invasion prior to surgery, since Pap and LEEP were not same reading. Good news is that there is no cancer.

But, results came back confusing - 2 out of 3 Pathologists read as CIN II. 1 of the Pathologist read as not CIN II (I don't have the medical terms) but dr explained that it might not even be CIN II at all...that it could be "reactive" (this might be the wrong word) And this could mean that I don't even have CIN II.

He says they sent the slides off to be re-read, and are awaiting more results. I have my pre op appt in 2 weeks where I will have my husband and can ask more questions. I always forget to ask questions on the phone when I am nervous!

When I asked if this could be that I don't have CIN II at all, and should I proceed with surgery, he said he still would recommend surgery, since every Pap will come back bad, and I will never know when it may turn into cancer.

I have not seen an Onc, since we were awaiting these results. Gyn said if micro invasion was confirmed, Onc would be next step.

I don't know what to think. I can't see having bad Paps every 6 months, and I never ever want a colposcopy again!

Has anyone else ever had this type of test result? Thanks for your help! I am so confused!

Re: Confused about colposcopy results

Hmm I would find that confusing too. I had cervical dysplasia and LEEP procedure before my hysterectomy too. But I didn't get mixed messages like you seem to be getting. Did you see your doc in person and discuss these results? I would tell him you need to know exactly what they found and what it means.

Re: Confused about colposcopy results

I do not know anything about your condition, but I am sorry you are getting mixed messages. That is never fun. You may try to get another opinion from a different doctor on your condition.

Good luck to you.

Re: Confused about colposcopy results

Sounds like you need a second opinion. That really would be confusing

I didn't have a colposcopy but my gyn-onc did say that pathology is as much of a science as an art. If you put the same sample in front of 10 pathologists and 7 of 10 may see early cancer and the other won't call it that way.

I would presume the same is true for CIN. I know with cancer if 1 pathologist tags it as that they usually go with that diagnosis. Since 2 of 3 said CIN II I would presume they will go with that but that is a guess on my part.

Re: Confused about colposcopy results

Sometimes cells can look odd/"reactive" due to other issues such as a yeast infection or bacterial vaginosis or just inflammation. Sometimes having sex the night before the smear is taken can produce odd results as well.

Also, PAP tests have a known high level of inaccuracy, and depending on where the sample is taken on your cervix, it could miss issues completely.

In 2007 I had a PAP come back as CIN III, I had a colposcopy and no issues were found. All PAP smears after this one came back clear. I thought I was out of the woods until cervical cancer came out of nowhere and here I am. I even had a PAP result come back perfectly fine on the day I went under to have a deep tissue biopsy done to confirm the cancer.

Re: Confused about colposcopy results

Thank you so much for this reply! I will track down the results as they were sent on for more review. My gut tells me to proceed w the surgery so I can put this behind me. God Bless.

Re: Confused about colposcopy results

Spoke to my regular gyn (not doing the da vinci, someone in his practice is) and found out that when they passed my pathology on the next person diagnosed CIN III. He explained that each pathologist may be reading a different biopsy (I had 4 during my colpo) SO CONFUSING!

Bottom line, if I want to put this past me I have to go thru with the hyst. I have processed this over the last 2 months, and I think I am ready to get this behind me!
