Need advice about recommended ablation and tubal
Hi I'm new to posting here. Do read here a lot though. I need some advice on how to proceed my gyno wants me to have an ablation. However, some of her answers to my questions contradict. I need a little "what would you do" help.
A little background. My grandmother had ovarian and cervical cancer (her uterus was precancerous when removed during hysterectomy. My grandfather had prostrate cancer. My aunt found precancerous cells in cervix when she was younger. My mom had a hysterectomy due to tumors (not sure if cancerous or just a lot). Precancerous cells were found after I had my oldest daughter 17 years ago. Had cervical scrape and no abnormal paps since. I'm 38. I'm having 2 periods a month lately. They were about every 3 weeks for a few months. I'm nauseated most of the time with odd burning abdominal pain. Sometimes I itch non stop (like bugs crawling on me). I'm bloated and sometimes look pregnant. I am over weight and can't seem to lose the weight, no matter my exercise routine and diet. Although I got frustrated and stopped the exercise and lost 10 pounds. In Feb 2015 I had emergency surgery to remove a 7cm cyst on my ovary it twisted so I lost my tube too. My gyno recently did ultrasound and my lining was 2.2x1.8x.3 cm. This was 1 week after last period and 1 week before I started my period. She did a biopsy but came out negative.
So here we are she told me that I need an ablation. She told me I have to get my tubes tied and gave me info for a research study (that her office is participating in) for an inhouse tubal and than wants me to do an ablation. She was adamant that I have to have a tubal because I cannot get pregnant after an ablation its not healthy. The pamphlet stated that you cannot have this type of tubal if you have one fallopian tube. I asked the doctor and a week later she stated that having the tubal wasn't necessary. Totally contradicting what she told me previously. My concern are my symptoms. If I have this, will I need to go back later for a major surgery because something else is wrong. What would you do? My insurance is so screwy righy now its not funny and I'm trying to get into see GP but it'll be a couple more weeks. How would you proceed?

I would first proceed with an opinion from a 2nd GYN. You have questions and have gotten mixed messages. (And personally, I would get much info before consenting to an in office tubal procedure). There may be other options, and this is not an emergency procedure, so you have time to investigate. Get all the opinions you need to make the decision right for you.

I agree getting a second or even third opinion might be almost mandatory. In office tubal ligation? Ummm, I'm thinking no Dr, Jekyll. I've never actually even heard of that.
I will tell you about my ablation. I had ridiculously heavy periods and the last one I had, I was in tears over it. I bled for 3 weeks time and it was heavy the whole time. I made an appointment to see my GYN. We talked options and what we thought. She suggested the ablation (let me add I had a tubal 20 years ago, so that wasn't part of the equation for me). I'm 50 and she said a hysterectomy now would throw me headlong into menopause, like a car driving 60MPH and hitting a brick wall. The ablation would slow, possibly even stop all bleeding and allow my body to go into menopause more naturally. I wanted a hysto, but what she was saying all made sense. So ok, ablation it is. It was scheduled for Friday July 17th, 2016. Day comes, surgery, success, go home and recover.
By Sunday there was a little blood. Oh no. By Monday I was bleeding. By Tuesday I was hysterical, called the office, I was almost right back to where I was. Because I could allow nothing vaginally, I had to wear pads, something I hadn't done since I was 14. That, too, was making the hysteria even worse. I thought this is the most vile, disgusting, unhygenic, blood filled disgustingness. My doctors nurse told me to come in on Wednesday and by all means, use a tampon if it would help to calm some of me down. I go in Wednesday, the doctor is going to take a look, get undressed. Ok, but I have to remove my tampon first. I go to the ladies room, come back, get undressed, wait for doctor. It is maybe all of 10 minutes. She says ok let's see what's happening. I say but first let me apologize, I'm sitting in a pool of blood already. She just lifts the sheet and is pretty shocked. She said I was borderline hemorrhaging and hysterectomy on Friday, July 22nd, 2016. For me, ablation was a total and complete failure. I am not sure I would ever recommend that to anyone. I have many friends that said it was a God send and they had no period afterward. It was not for me.
Please get another opinion and make sure what is right for your body is what is done. Best of luck.

Oh my. That is scary and one of my fears. One of my friends swears by her ablation but my aunt was saying hysterectomys in my family. I am going to my GP to get a 2nd opinion. I am so sorry you went through that. I hope you're doing better.

I am actually doing great, thank you for asking. The ablation was a failure and my emotions were ridiculously out of control. But the hysterectomy has been fabulous. Easy recovery, no more bleeding, I'm feeling like a new woman or at least the woman I used to be before the problems began. I hope you get the same very quickly.

Keep in mind that a hysterectomy does not include ovary removal (so you would not go into menopause at age 38). Ovary removal can be done at the same time as a hysterectomy, but most women do not remove their ovaries unless there is a problem with the ovaries or a family history of ovarian or breast cancer. That being said, Whoville since your grandmother had ovarian cancer, you should at least discuss with your doctor if you should get genetic testing to see if you're predisposed to ovarian cancer and whether it makes sense to remove your ovaries at such a young age.
Your doctor's discussion about in-office tubal is probably the Essure method of a tubal "ligation" via the Essure implant, which is placed in-office. A lot of people have had issues with the implant and end up needing a tube removal &/or hysterectomy to remove the implant, so please explore that carefully if you're considering Essure.
It's always a good idea to get a second or third opinion before undergoing any sort of invasive or permanent medical procedure or surgery. It doesn't sound like you are in an urgent situation, so please take the time to explore options and find out what is best for you! Good luck!

If you are concerned about uterine cancer an ablation might not be the best option because it causes scarring that can affect future biopsies and mask any potential cancer. If you are past childbearing I agree that a 2nd opinion is in order and I would recommend a gyn/onc for the 2nd opinion. I was post-menopausal with very heavy bleeding that both Progesterone and D&C's did not help. My gyn was loath to do surgery because I am overweight. I went to a gyn/onc in a larger city and he offered me robotic surgery and it went very well. Another lady suggested keeping one or both ovaries and that is also your decision. But a hysterectomy is doable and so much better than the constant bleeding.......which for me was a major quality of life issue and also caused severe anemia. I hope you find a solution.

Hello . I haven't read through the other answers on this board but wanted to tell you my experience. I am similar to you in that I have family uterine cancer history. My mother also had a very large bowling ball sized fibroid removed from her in an emergency hysterectomy. I have had numerous problems as well and lost my tube to an ectopic pregnancy. I have a history of severe endometriosis which spread beyond my uterus and attached into my digestive tract. I was hemorrhaging every month with severe heavy periods. My doctor gave me the option of a tubal/ablation or a hysterectomy. At the time my girls were just 2 and 4 and the ablation surgery was supposed to be a much shorter and easier recovery. So I chose to do that instead of the hys. On the day of the tubal/ablation I went home and did not have to stay. I was in excruciating pain. It was like child birth cramping times 20 and was bleeding. I went home with a rx to Percocet and that helped some but I sat on the couch with the heating pad miserable for over a week. I looked 8 months pregnant I was so bloated and swollen. After I healed up from the procedures I felt better and the bleeding from my monthly was next to nothing. I was amazed at how well the surgery worked but that was short lived. About a year later is when the problems began. I started having a shooting pain in my right ovary (the ovary with the tubal block). It would shoot pain so severely it would stop my in my tracts and throb out of my vagina. It was excruciating. I also began bleeding pink/brown daily and had to wear a pad all the time. Then the odor began. I would have a terrible odor evey single month after ovulation. It was so embarrassing and i had never experienced anything like that before. I am huge on cleanliness and hygiene and so when I noticed an awful odor up inside my vagina it was devastating and humiliating to me. I didn't want to have sex with my husband because of the pain, bleeding, and odor. It continued on until my dr and I agreed I needed to do the Hysterectomy. Last Tuesday I went in and had a robotic hysterectomy and oophorectomy. They took everything. I already feel better at 1 week than I did at 2 weeks post ablation. In my experience I would skip the ablation and just do a hysterectomy. It will be over and you won't have to worry about it or another surgery. I have been on estradiol and have had no issues with hot flashes or emotional problems. I feel great. The pain was less than the pain I had with the ablation and tubal. Please get a second opinion!

I had an ablation in 2004 because of horrible periods that lasted 2-3 weeks at a time. Ten days later it would start all over.
Everything was great until last year when I started having abnormal paps and heavy bleeding. My uterus is so scarred from the ablation that the doctors can't see if there are any tumors in there.
I'm having a hysterectomy on Sept. 30th because at my age I don't need my uterus. I'd rather be safe than sorry cancer wise.
