The Difference Between a TAH and a TVH
Abdominal hysterectomy?
An abdominal hysterectomy is a procedure in which the doctor removes your uterus through an incision (cut) in the abdomen. There are two types of abdominal hysterectomies: total abdominal hysterectomy (TAH), and supracervical abdominal hysterectomy (SAH). In a TAH, the uterus and the cervix are both removed. In a SAH, only the uterus is removed. Although your ovaries may be removed during a TAH or SAH, these procedures can easily be performed without removing your ovaries.
To perform the surgery, the doctor makes an incision in the abdominal wall to expose the uterus and adjacent structures (e.g. ligaments and blood vessels). The doctor separates the uterus from these structures, tying off the blood vessels so they will not bleed. In a SAH, the doctor removes the uterus where it attaches to the cervix, such that the cervix remains in your body after the surgery. In a TAH, the doctor removes the uterus and cervix from the vagina. After a TAH, the top of the vagina is repaired so that a hole does not exist between the vagina and the abdomen.
During a TAH or SAH, the doctor may perform additional procedures. These may include removing the ovaries and fallopian tubes, shortening the ligaments that support the vagina, supporting the bladder, and repairing any anatomic defects that might be present. If you are going to have any additional planned procedures, your doctor will discuss them with you prior to surgery.
vaginal hysterectomy?
A total vaginal hysterectomy (TVH) is a procedure in which the doctor removes your uterus and cervix through an incision (cut) in the vagina. There are no abdominal incisions with a TVH. Generally, you experience less post-surgical pain, and recover from surgery more quickly, if you undergo a TVH as opposed to an abdominal hysterectomy. Unfortunately, not all women who are candidates for a hysterectomy may have a TVH - the decision as to whether a TVH is appropriate must be made on a patient-by-patient basis. Although your ovaries may be removed during a TVH, a TVH can easily be performed without removing your ovaries.
To perform the surgery, the doctor makes an incision in the vagina to expose the cervix and uterus, and the adjacent structures (e.g. ligaments and blood vessels). The doctor separates the cervix and uterus from these structures, tying off the blood vessels so they will not bleed. After the cervix and uterus are removed, the doctor sews some of the ligaments to the vagina, and then sews the top of the vagina closed so that a hole does not exist between the vagina and the abdomen.
During a TVH, the doctor may perform additional procedures. These may include removing the ovaries and fallopian tubes, shortening the ligaments that support the vagina, supporting the bladder, and repairing any anatomic defects that might be present. If you are going to have any additional planned procedures, your doctor will discuss them with you prior to surgery.
