Just wanted to let you know that I was having problems with my insurance company also. They asked me to write what my symtoms were and how they affected my life. Well, I think they got more then they needed. I found out last night at 9:50 pm that my insurance company decided to cover it. Maybe you should write your own letter to your insurance carrier. If you do be VERY detailed. I'm 36 and I have 3 children. I told them I'm tired of feeling 66 instead of 36 (No offense to anyone with that, just getting across I don't feel my age) because of bladder and bowel problems, back pain, reflux and of course the pain and severe bleeding associate with my problem. When the doctor called last night to tell me, she laughed and commented on the fact that I let them know what was going on and it worked. She called it an "essay" but it got the point across. I'm still not 100% sure I'm ready for the hyst. but at least now the ball is in my court. Best of luck and make sure you let your voice be heard. You know what they say, "You are your own best advocate!"
