Thread for Pre-op questions

I have my Pre-op next Tue (surgery will be May 26 - 6 weeks on Monday & counting!) - I remember seeing a thread about which questions to ask my GYN but can't seem to find it. Can someone please point me in the right direction?

I started with this list and then added some questions that I gathered along the way from reading different posts on this site.
