Only have 3 months to decide......

So, I am one of the millions of Americans that cannot afford the $700/month health insurance, but I make more than the state requires for help (if they are even helping at all at this point). My Dr. referred me to a program where surgeons volunteer their time for people who need surgery w/o insurance. The gyno they sent me to said I have a few fibroids and he gave me a 2nd depo shot (progesterone). He wants me to wait 6 more months to see if it helps with the excruciating pain I have every cycle.
The problem is, you are only in the program for 6 months and I have 3 more to go. This will be my 5th month on depo. If I have awful, painful cramps this month, should I insist on hyst?
I am a 40 year old single parent with no desire for more kids.
