All the issues you are worried about are completely normal.
My surgery may be over an hour away as well and my husband does not drive either (he is disabled).
For other surgeries, we have relied on friends or taxis. We are fortunate to have a service in our town called Senior's Home Support that will also cover the disabled or others in need of help for medical reasons. They will arrange for people to do the services (they screen the people) and we pay the people. It works for drives or yard work or snow shovelling, etc. It is worth investigating if there is any such service where you are.
By the way, even the avoiding is "normal" (I hope). I have at times been depressed about my situation and just "lurked" for sometimes a couple of days, sometimes more than a week.
I will keep you in my thoughts and prayers.
