Questions on using Premarin cream

Hi girls!
I'm going to warn you right off the bat that this post will not be for the faint of heart!

OK - so the doc started me on premarin vag cream last week for some vaginal atrophy - I was starting to have some pain with intercourse and burning urination and what not. Everyday for two weeks then twice a week - 1 gram.

Anyway - so I start this on Monday and had started some metrogel the same day as I had BV. SO no sex for Dh and I during the week. Saturday I skipped my premarin dose in hopes of some leisurely sunday am sex with hubby without the mess, etc. You all know what I mean!

Anyway - Sunday morning we have sex and it is a mess like I've never seen!!! I mean frothing, huge clumps of premarin cream all over dh and me, it was quite the scene!! Then, we proceed to have sex again in the evening (lucky me! :O) !!) and it's the SAME deal!

What gives? Isn't this stuff supposed to be absorbed into the walls of the vagina?

My major concern (other than the MESS factor, of course!) is that the pamphlet in the premarin cream package said to avoid having sex right after insertion of the cream to reduce my partners exposure to and absorption of the estrogen through the skin of the penis, etc, etc. Well, if it's still in there two days later, how on earth are we ever going to have sex without exposing him to this estrogen and having this frothy chunky mess??

I should point out that I can't use systemic estrogens because of cancer risk, so we were going this route because there is minimal absorption.

Is this normal during the first two weeks of daily dosing? Does it get better when you are on the twice a week dose? Any other thoughts??

Jo, that sounds really bizarre. Could the two vag. treatments be reacting together to form the weird frothy clumpy stuff? I am using an Estring for vag dryness and the instructions clearly state to NOT use while being treated for a bacterial infection. A call to a pharmacist would clear that question up right away.

BTW, the Estring is a nice alternative. It stays in for 3 months and delivers a slow and tiny dose of estradiol, the same stuff your ovaries make naturally. Patients who can't take HRT are able to use this, as it delivers the tiniest dose to the tissues where it is needed w/out raising blood levels. The first week of wearing it, I could kinda tell something was there and dh could feel it during sex, but now it's like it's not even there.

I agree with the other girl that it sounds like some sort of reaction between the 2 meds. Put a call in to your pharmacist; he/she will know right away whats going on.

Thanks for the thoughts, ladies.

I asked the doctor about it when he prescribed them both and he said no problem. I am going to call him today though and ask about the foamy/frothy mess and see if perhaps I would be a candidate for that little pill thing that gets inserted into the vagina instead of a cream.

Hubby and I are going to Vegas this weekend to celebrate our anniversary and I'm at a toss up about what to do! I want to be able to have lots of lovemaking, but don't want the mess, but without the cream, sex is going to be painful! Grrrrr.

I'm hoping the pill/tablet thing will be an option.

Any other thoughts?

The symptoms you are describing with the frothy discharge are really more consistent with BV and not from the Premarin cream. I would really suggest you get checked out to be sure your BV has been fully treated. You and DH may be passing it back and forth. Most doctors will treat both partners to keep it from being passed back and forth.
