Hi everyone,
I just had my surgery Friday (TAH/BSO). I chose hysterectomy when I was diagnosed with complex endometrial hyperplasia without atypia. When my doctor visited me on Saturday he said that it was a good thing I had the surgery, because upon opening me up he found I had "severe endometriosis" and that one of my ovaries was "the size of a tennis ball."
I see my doctor on Friday to have my staples removed, and I'm starting to put together my questions to ask about the future. (What should I be monitoring, what things may return, etc.)
Since I wasn't expecting the endometriosis diagnosis, I'm a little lost. I've done some reading here and at WebMD, and I can see how endo would have been a factor in some of my symptoms (severe menstrual pain, constipation during periods, pelvic pain). I'm not seeing anything from either condition that explains why one of my ovaries was enlarged. Also, is there any connection between endometriosis and endometrial hyperplasia?
I'm sure the pathology report and my doctor will be able to answer some of these for me, but I was wondering if anyone here has had a similar combination of diagnoses? Any other questions I should be thinking about?

With the diagnoses of endo you need to be aware that estrogen feeds the disease so going on ERT can aggravate it. And even if your doctor thinks he got all of it at the time of the surgery, some of it can microscopic and not seen during the surgery.

I had hyperplasia without atypia, and they had done a d&c on me a year ago, but it didn't help. When I had my TAH, they told me I had all kinds of problems -- fibroids, an enlarged uterus (not solely due to the fibroids, they couldn't explain why it was so enlarged - I think he was referring to the overall thickness - other than they said something about the muscle being overdeveloped?), cysts on my tubes, ovaries adhered to my uterus, and endo wrapped around my bladder and bowels. They said my bladder had the most adhesions. My doc gave me a choice, but informed me first -- if you had endo and take hormones, it has a better chance to come back because it feeds off the hormones. My problem is that I'm overweight, and excess hormones store in the fat, so he said it would be more than a year before I am rid of the excess hormones in my body. I decided not to take hormones unless I couldn't survive the symptoms of menopause (I had already gone through medicated induced menopause the last year anyhow). So far, so good. I have a few emotional moments, hot flashes, times when I can't get my cold feet warm, but other than those infrequent things, I am doing well.
If it were me, I would simply ask if the pathology was clear, what exactly they found, and what it means for your future health. I have found that sometimes they cannot explain why (for example, why my uterus was so much larger) because it was just something that happened as a result of all the other issues. Since my TAH, I can tell you that all the other problems I was having (bowel, bladder, high blood pressure, etc) have subsided as of 2 weeks out. Good Luck to you.

Thanks for the replies. This is helpful info.
I was going to try and get by without HRT anyway, so this just gives me more incentive. (My aunt died of breast cancer that she always felt was triggered by HRT). Because I'm overweight, I was hoping that the additional estrogen produced by fat cells might mitigate symptoms enough that I could get through menopause without it.
I haven't started reading in the Hormone Jungle yet, but I figure I'll head there next. Thanks again!
