What questions should I ask my doctor?

I have an appointment with my OB/GYN tomorrow morning to discuss my upcoming (July 5th) surgery in detail. What questions should I ask him? I want to write them down so I remember what to ask when I get there. My surgery will be a TAH for a very large fibroid (20 weeks). He plans to leave my ovaries, but we need to discuss my cervix (whether or not to take it). Any help I can get from the experts who have "been there, done that" would be a great help to me. Yep, that's the right place to start for a list of pre-op questions. And of course, add any of your own that aren't on that list...that might be specific to your condition, your lifestyle, or your home.
