SAH with unilateral oophorectomy for post-ablation tubal sterilization syndrome
I am trying to figure out how to respond or post. I think I've accidenctally found it.
I'm due for a SAH with unilateral tube and ovary removal for suspected post ablation tubal sterilization syndrome.
Has anyone had any experience with this particular disorder and if so please tell me your hysterectomy fixed it.

I would love to hear your symptoms and how you were diagnosed. I am having problems and have had an ablation and a tubal. Please fill me in.

I have had extremely heavy periods since my tubal 14 years ago. Finally I went to a new OBGYN and she suggested an IUD. Had a nightmare experience years earlier so said no. Then they suggested an ablation. June 07 was my surgery and all went well until my first period which landed me in emerg. with pains that were worse than any pain I've experienced. They thought it was a kidney stone and shot me up with meds & released me. It continued that month and every month since. Sharp incredibly painful knifelife pain around my left ovary. (similar to an ovarian cyst rupturing but it stays for 4-5 hours at least). My periods continued to be heavy enough to wear nighttime pads in the day and I continued to soak through my clothes.
In Nov I had a diagnostic laparoscopy and it showed beautiful internal organs. My dr. thought maybe she had burned a little too far down with the ablation so she opened my cervix to allow the blood to go through. Didn't work. Pain is still there every month and most times I end up in emerg being shot up with morphine.
Finally I met with her again determined to have no more experimental treatments. I researched everything myself as nothing has shown up on Transvaginal US or CT scans (although they only looked at one area with the latter test).
I typed in my symptoms etc and found two possible scenarios. The first is adenomyosis (endometrial tissue between the muscle layers of the uterus) this is actually what my doctors interns suggested it might be and it cannot be diagnosed without looking at the uterus in pathology, the second being this post ablation tubal sterilization syndrome.
Basically the pain hits in the very low abdominal region and it feels as though someone has their hand on my ovary and is twisting it and trying to rip it from my body. It most always starts later in the day but that depends on my flow. It only happens during my period and continues for 8 days. Last month the pain came back during my ovulation time so I know for sure it has something to do with my ovary. I've asked for her to remove that ovary and tube as well. I'm not taking any chances. It is way too painful and too hard to give up a week of your life every month to this kind of pain.
As a side note, my doctor figures it is highly possible that this is what I have although she has never seen any cases of it in her practice.
What happens is that the tube (which has been surgically tied off) fills with blood from the uterus causing symptoms similar to an ectopic pregnancy. Sometimes they can tell if they are looking inside while you are on your period. Apparently it will show a swollen tube but that's not a definite.
Either way, I'm having this done and praying that it works because I really don't know what else it could be and there is a 3% rate of this happening after an ablation because scar tissue adheres to the tube and makes it worse.

Sande5, what you have described is quite similiar to the pain I get,(although my pain is also up in the kidney area), but I only get it at ovulation as I had a TAH in August 07. It started occurring at the time of my periods which is what lead me to a TAH. I have been having these pains since my Tubal ligation in 2000. I had "clips" put on my tubes.
I am going back to my GYN today and am going to ask that the clips be removed and see if this will resolve things. I am not keen on ovary removal as all tests have shown over the last 8 years that the ovaries are "normal" and I would like to avoid surgical menopause, but if it is guaranteed to stop the pain, I would probably do it.
The TAH went very well for me and I recovered quite quickly and was back to myself in 6 weeks. I have never regretted having it done once. I hope you have a speedy recovery and hang in there until May. I will be following your posts to check up on you. Good luck.
