Got medical records

I finally got my medical records. They contain many things which are not accurate. I guess I am just amazed at how different they are to my recollection.

Just for an example they tried to do a hyterscopy (sp?) in the office. It was the worst pain in my life. I pretty much ran out of office bawling and cried most of the day. She was not able to get through my cervix. The records say I handled procedure well with some mild cramping. Huh???????

One day I had called because I had so many clots they were coming out like a water faucet. The records say "patient complains of watery discharge"

Its all goofy stuff like that.

So the question is when I send them to the 2nd opinion gyno do I make notes on the records?

I know how frustrating that can be. I got copies of my son's records to a specialty referral that had some significant errors. Technically ~ you should ask the office to correct them and then provide a corrected copy to you.
In the case with my son, I did not have time to do that so I drew a single line through the error on my copy (so what was written by the doctor could still be read) made a side note correction with my initials next to it. Then, at bottom of page, I wrote: Corrections made by: my signature, relationship (patient, parent, etc.) & date.

If you do end-up making the corrections yourself, you could provide your original doctor with the 'patient-corrected' copy, and ask them to place in chart for you to discuss at your next visit, so the doctor will know what areas you feel need correcting and what is an accurate version to you.

How do you go about getting your medical records? Just ask? Cost? I need to get mine & am not sure how to go about it.

I Had to go to the office and sign a release. They gave me the option of sending them to the doc or mailed to me. I had them sent to me.

There was not a charge. I have also learned you can request a copy of the record when you have a visit. Haven't tried this yet but from what I hear you can have them mail you a copy of the days visit (record)
