I know how frustrating that can be. I got copies of my son's records to a specialty referral that had some significant errors. Technically ~ you should ask the office to correct them and then provide a corrected copy to you.
In the case with my son, I did not have time to do that so I drew a single line through the error on my copy (so what was written by the doctor could still be read) made a side note correction with my initials next to it. Then, at bottom of page, I wrote: Corrections made by: my signature, relationship (patient, parent, etc.) & date.
If you do end-up making the corrections yourself, you could provide your original doctor with the 'patient-corrected' copy, and ask them to place in chart for you to discuss at your next visit, so the doctor will know what areas you feel need correcting and what is an accurate version to you.
