24mm thickened endometrial lining and worried with questions

Hi, I'm new here but have been reading through the forums for the last few days. I hope someone can offer some insight into my mess of a story.

I am 37 and have been having issues with my period for as long as I can remember. I have very regular cycles but terrible pms, cramping, heavy bleeding (sorry for the tmi).

I have also always had other health problems. Last month I wound up at the ER because of stomach problems.(pain in the upper right quadrant, indigestion, diarrhea, lower back pain) I have been dx'd in the past with IBS but I've been seeing a GI specialist because they are trying to rule out Crohns as well. I also have Fibromyalgia, hypothyroidism, and some other non diagnosed (yet) autoimmune problem. We have four children and two are special needs so life is pretty chaotic all the time.

Ok so for the last year or so, my periods have been getting progressively worse. For the last 6 months or so, I've been spotting in between periods, after a BM or after being with my husband. I am also spotting for days before I actually start my period. When I do start, I have cramps that could bring down an ox and I pass some pretty scary clots. I also wind up having a hard time with my IBS around that time too (lots of diarrhea) and extreme tenderness in my pelvic cavity. Last week I went in for my pap and my Dr. sent me for a transvaginal u/s. The Dr. who did the U/S said he saw no polyps, no signs of adynomyosis (sp?) (which a Gyn dx'd me with a few years back), and no fibroids. He said my lining, however was 24mm thick. I'm going to see an Ob/Gyn this week to get to the bottom of this but I'm nervous. First of all, a few years ago I was dx'd with low progesterone (we have been trying to get pregnant again for the last 9 years and have been unsuccessful). I was also told based on my heavy periods, pain, etc a few years back that I probably had adenomyosis as well. (no treatment at the time as the Dr. said if it got worse, we would have to do a hysterectomy).

I'm concerned because I'm wondering if all the sickness I've been dealing with for the last year or so has actually been something more serious? I'm having lots of lower back pain, terrible sciatica pain right now, frequency in urination (sometimes it feels like a UTI is starting but it's not), lots of pelvic pressure (sometimes feels like there's a bowling ball in my uterus). I just have a bad feeling about all of this. I am trying to find out if my grandma (who died of breast cancer) had uterine cancer when she was young. My mom thought she did but no one can confirm for sure.

Anyway, I was hoping to get some input from all of you here. If I go to this new ob/gyn this week and they dismiss it, should I insist on a biopsy? (since I'm young, I've heard Dr's dismissing the possibility of uterine cancer). Also, has anyone btdt? If it's adeno or endo, has anyone had a hysterectomy for those reasons?

I would expect a biopsy at this point, based on the ultrasound, but I'm no GYN. I wish I'd had a biopsy the first time my ultrasound was borderline, although who knows what it would have shown. It was a few yrs later I had the biopsy that showed cancer but I always wonder if it could have been found at a pre-cancer stage. So of course I'm biased as to having a biopsy. I hope your Dr visit gives you some answers. Its hardest wondering and worrying, I think.

Thank you I got my appt. for this Thurs with a Dr. I've never seen before so hopefully she will be good and thorough.

Do you mind me asking why your U/S was borderline and how old were you when you were diagnosed? I appreciate your taking the time to respond. This is definitely scary and my family is blowing it off as being "probably nothing" which it very well may be but it doesn't stop me from worrying.

My first ultrasound was just at the edge of normal/abnormal and they tried to get a biopsy but couldn't. They said I could have another attempt or noT, so I chose not. I was perimenopausal and having spotting but eventually it stopped. 4 yrs later I had post menopausal spotting and an abnormal thickening on ultrasound, cancer on biopsy. No way of knowing what, if anything, an earlier biopsy would have shown. I think its standard to biopsy a thickened endometrium but it isn't always cancer, it can be other conditions. I hope you get some answers soon. Worrying is the worst.

Well I just got back from the Dr. I had the biopsy (ouch!!!) She seems very nice. She told me that if it's hyperplasia then we can treat with hormones. She said if it's hyperplasia with atypia (s?) cells then we can still treat with more aggressive hormones or talk about a hysterectomy. She seemed to describe the atypia cells as "pre cancerous" ??? I thought hyperplasia was pre cancer and atypia cells are "cancer"??

Anyway...now I wait. The earliest the results will be back will be Monday...otherwise I have to wait til Thurs (she's off tues and weds). Let the worrying and waiting begin

Waiting is the pits. I'm glad you had the biopsy. In this case hyperplasia with atypia is considered pre cancer, I'm not sure about terminology for other cancers. I'm hoping you have the shorter time to wait. Let us know what you find out.

Thank you so much for replying. This has been one of the most stressful times of my life (and I've had plenty!)

I read the U/S report and it had stated that my previous U/S done in '07 showed I had a 17mm endometrial lining. No one ever told me this! I don't remember which Dr. ordered that one because I was going to a family practice with several rotating Dr's and NP's.

Can I ask one more question? If the biopsy comes back as not hyperplasia, then, do I still need to monitor it closely? Does a chronic thick lining increase the risk of endometrial cancer?

I'm no expert but I thought a thick lining usually was hyperplasia unless there was a polyp. I think all hyperplasia gets monitored closely since it can progress to cancer. So probably they'll monitor no matter what shows up. I'm not sure how fast it can progress, I know it tends to be slow. I hope you find out Monday.

Just wanted to update....I got the results of the biopsy and everything is fine. No hyperplasia! The Dr. told me some women just have thick linings?

I told her I was happy but still nervous since it still doesn't explain the other symptoms (cramping, bleeding, constipation, etc). She is going to start me on BCP for 3 months to see if it thins it out a little and helps alleviate symptoms. I HATE the idea of taking them but I also am desperate for relief!

She told me if it's not better, we can talk about a D&C in a few months but she said it will most likely grow back. I'm happy but still nervous. I've read so many stories here about normal biopsies followed by abnormal D&C biopsies. Should I just take this as a "phew, but I should really work on my diet and stress" or should I still worry?

That's really good news. Its true a D and C is definitive so if things persist that should show any problems that are still there. I guess if I had doubts I'd get a consult opinion from a GYN oncologist to see if I got the same approach. Then I'd say whew and just follow up. It doesn't really help the problem that its all an OK thing but mentally----"no cancer" are some sweet words to hear. I hope it gives you some peace of mind.
