Fibroids - When Is a Hysterectomy Necessary?

From the Ask a Doctor - Hysterectomy Related Questions Articles List

When is a hysterectomy necessary for treating fibroids? Does it matter what kind or how many fibroids I have?

Reply by James Kondrup, M.D.:

A hysterectomy is necessary for treating fibroids when symptoms from the fibroids become bothersome or when the amount of uterine bleeding is significantly affecting your life. A hysterectomy isn't your only choice, however, especially if you want to preserve your fertility.
Hysterectomy:
This is usually performed when you are done having children, the fibroids have continued to grow, and symptoms include bleeding and pressure on the bladder, pelvis, or back. If you do need a hysterectomy, it can almost always be done laparoscopically using small incisions.

Uterine Artery Embolization (UAE):

In this procedure, a specially trained radiologist called an interventional radiologist passes a catheter into the uterine artery, and then uses plugs to block the artery to the fibroid. This starves the fibroid of blood and causes it to shrink.

Myomectomy:
This term refers to removal of the fibroid. This procedure is done for those woman wishing to preserve their fertility, or who are adamant about not losing their uterus. This is an advanced procedure which can be done laparoscopically by an experienced surgeon.

Hysteroscopic Myomectomy:
This procedure involves removing the fibroid by shaving it with a device that has been passed into the uterus through the cervix. Hysteroscopic myomectomy is done when the fibroid is located inside the uterine canal.
Many factors are involved with fibroids and how to remove them. This includes the size, the number, and their location. Your surgeon may want you to have an MRI to better assess these factors and determine the best treatment option for you.
