Complex hyperplasia w/o atypia after ablation

I had a uterine ablation in July. When I went in for my follow up, my doc said the pathology taken during the d&c turned out to be complex hyperplasia without atypia. He is very concerned because he says the ablation can mask hyperplasia and cancer. So he told me I can have a hysterectomy right now, which I didnt want to do, so he's given me 6 months on provera before I return. I stopped taking the provera d/t I hate the way I feel. Last week, I got a second opinion from a gyn onc, who also strongly recommends that I have the hysterectomy. Has anyone here had this experience? I'm so scared. I don't want to have it, but I don't want to find out in 5 years that I have a cancer I could have prevented. I'm so sad that they didn't find this before the ablation. Basiclly, both dr's are saying i would have had more options if not for the ablation.

Re: Complex hyperplasia w/o atypia after ablation

Sorry I can't help much. My gyn refuses to do ablation at all for that reason. He said the scar tissue could mask cancer in the future. Scary I know. I'm surprised your Dr. did the ablation without knowing the results of the d and c. Maybe you could get a third opinion. I know your in a terrible place right now but you have the support of everyone on the board. Hang in there.

Re: Complex hyperplasia w/o atypia after ablation

Thank you for replying. I had an in office biopsy before the ablation, it was clear. He did the hysteroscopy , d&c and the ablation in one sitting. After all of that is when I found out. Thank you for your support, I've yet to find anyone else in this particular position. I'm scared and upset, but I know that normal

Re: Complex hyperplasia w/o atypia after ablation

Very similar story to me...I had a DNC, hysteroscope and Novasure ablation done on July 5th after my periods became unbearable after my c-section last December.

A week and a half later I got the call about hyperplasia being found and that because of the ablation, hysterectomy was the recommended treatment.

I never healed from the ablation (path from hyst showed an infection had set in), the ablation was a big failure, as I now had horrendous labor like cramping all the time and was getting my period every 21 days. I also had a cyst rupture in August that sent me to the ER, but that's a whole other can of worms.

I ended up changing doctors, went to a larger medical center that has the DaVinci robot, and he gave me two options...hysterectomy or hormone treatment with biopsies under sedation in the OR every 3-6 months because he would not be able to get an accurate biopsy in office with the scarring from the ablation.

Hysterectomy sounded like the better option to me then the biopsies!

My final path from the surgery showed that not only did I have an infection from the ablation, the hyperplasia had returned and was embedded in the uterine wall and would not have been found with a regular biopsy.

I'm am thrilled that I no longer have to worry about it turning to cancer and me not knowing.

I had several other issues that the hyst treated, not related to the hyperplasia, but I would have have the surgery either way because of the ablation masking possible cancer.

Thanks for sharing with me.. I'm sorry you had to go through that, but it sounds like you're much better. I know I will have to go through with it, but I guess I'm just scared and upset, just like everyone else. I get nauseated every time I think about it.

Re: Complex hyperplasia w/o atypia after ablation

If it would make you feel better about the decision, by all means...get a 3rd and even 4th opinion. I did not have an ablation, but....I do have experience with the hyperplasia w/focal atypia. Mine was caught early enough - no cancer found... and for that I'm grateful. My Mom and one of my sisters both had uterine cancer, hysterectomies....etc. My Mom had to have radiation, my sister did not. So...I'm truely grateful I don't have to worry about it anymore. And....mine was all caught because I herniated a disk in my back, had to have an MRI and thats when my journey began. Otherwise, I may not have been so lucky if it hadn't been caught at an early stage. Yes, I did say lucky...no cancer = lucky in my world. Good Luck with your quest for a workable alternative - I truely hope you find what you are hoping for!

ps...I'm almost 4 months post op...had a very easy recovery and am doing wonderful, back to my normal self and activities!

Re: Complex hyperplasia w/o atypia after ablation

I am so sorry that you are going through this, and if I had not read your story, I swear that it was mine you were telling here on this site! I had EXACTLY the same experience as you, but i had my ablation in March. It was all done due to heavy/daily/soaking bleeding that would not stop. My biopsy's also came back clean with 2 D&C's prior to the ablation and I also had about 30 egg size polyps removed as well. 6 months later I was still bleeding, and we had moved to another state due to my hubby's job change. I found another specialist and she said the exact same thing as my former doctor which was if this did not work, a hyster may be the only option. I did know that that the ablation could mask underlying cells and we tried everything and every route we could to make my uterus behave. The thing that did it for me, as I have now had 6 procedures including another D&C just 6 weeks ago (again clean biopsy) were my ultrasound results, both abdominally and vaginally. I just turned a young 50 and both of my doctors said that my uterus should be about 5 (give or take a bit) mm, well after 6 procedures, mine is almost 15 again and both of my MD's said that puts me at a 50/50 risk for uterine cancer down the road in maybe 10-15 years. For me, my decision was made right then and there to take it all out. I am tired of bleeding everyday, the cramps etc, so out it goes. They are leaving my ovaries and I feel great about the decision. I don't want to take a chance that this will turn on me and to do this at age 60-65 would be so much harder then it will be now. I am very active, healthy, and I think I will be much happier. I love my uterus for giving me two great daughters, but it's now time for her to go as she cannot behave and I can't and won't live with the chance of cancer, when I can prevent it. I am ready to do this, and I feel good about my decision, though I know how hard all this is on you. My biggest worry now is the recovery and hoping that my personal life with my hubby will be the same after this is all done. Hang in there- get more opinions if you need them, but this is a great site for support and information. Best wishes and I will keep you in my thoughts and prayers! Many hugs sent your way xoxoxoxo (sorry for the long post)

Re: Complex hyperplasia w/o atypia after ablation

I appreciate all of the support I'm 36 and I worry a lot about if things will go down hill for my husband and I. I've read good and bad stories. It sounds to me that hysterectomy is the suggested course across the board, now I suppose I just need to wrap my head around it and all that it entails

Re: Complex hyperplasia w/o atypia after ablation

Make the decision based on what is best for you and your hubby. By NO means was it easy for me as I wanted to try every other option available first. However, nothing worked as we had all hoped, and once I saw the results of my ultrasounds and did my research, I felt I had no choice. I'm not scared of surgery, I'm scared of the recovery and I don't want my sex life to change with my husband because it's great right now. However, that being said, for me now it's come down to quality of life and not taking any more chances as all other options have been exhausted. It's not easy at all, and yes, it is overwhelming. The other lovely 3 ladies above have all been where you are, and I am as well. You are doing fine, just keep thinking logical, talk to your doctors. This is a great site for info and support. If I can do anything for you at all, I will. Best wishes and hugs galore. xoxoxoxo Elizabeth
