Heavy bleeding & fibroids
Hello there,
I have an appointment tomorrow with a gynecologist and I wanted to know what you all thought.
I have been having heavy periods for seven years. I'm 45 now. I have been bleeding since last November every day. I have five fibroids. One is pendunculated on the fundus, about 5cm, one is another pendunculated one, about 4cm. One is intramural, about 3cm and then I have two submucosal ones that are about 2 and 3cm.
I am tired. I have urinary frequency. I am tired of bleeding every day. My once flat abdomen sticks out and I feel bloated all the time. I have been offered Mirena and am currently taking birth control pills. I should have seen about this sooner but my father died last month and it was a month wait for the gynecologist appointment. I read stories of people with far larger fibroids than myself. I wonder if I am jumping the gun, but I do not want to have the Mirena IUD.
Is looking for fibroid cauterization or a hysterectomy with keeping ovaries intact a little drastic?
Any words of wisdom?

Hi, I just saw your post. I'm sorry about your father and what you're going through with your fibroids.
I had an abdominal myomectomy last year for a very very large fibroid (cantaloupe to honeydew melon-sized) and several smaller fibroids. I chose a myomectomy because I only wanted the problem removed (the fibroids) and not otherwise-healthy organs.
Two pieces of advice:
1) Get SEVERAL physician opinions before you make a decision. Seek out the best doctors for these opinions, even if you have to travel a ways to see them. Remember, a lot of doctors will recommend a procedure because they are comfortable doing that procedure, not because it's necessarily the best procedure for YOU.
2) Keep close tabs on your iron levels, since you're bleeding every day. Make sure you take the appropriate supplements while you're deciding what to do. You don't want to become anemic, especially before a possible surgery.

Hello Twysmiling and LaurenAbigail.
I was told that because of the large pendunculated fibroid on the fundus that fibroid cauterization couldn't be done on that. Which is a shame as that's what I was hoping for.
This gynecologist was a little cavalier in my opinion. She said that if they took my ovaries too I'd have less risk of ovarian cancer. I'd rather leave my ovaries intact.
I have < > insurance and she did give me a list of other gyneocologists, however there always seems to be a month long wait to see one. So I've booked two other appointments. I don't want to go with this lady's opinion.
I've also realized that I should read this board a lot more. LaurenAbigail, I'm going to take a look at your post about the myomectomy.

i'm glad you are getting more opinions. if your ovaries are healthy there is NO reason to take them out and put you into surgical menopause. her reason is very poor. good luck with your other appointments!

I was embolized and it worked great! Fibroid has shrunk 90%. You need to speak to an Interventional Radiologist because GYN's are NOT well versed in the particulars of UAE. Only an IR can determine if you are a candidate as it is their specialty. I know of a lady with a fibroid on a stalk who was embolized and she has had tremendous shrinking too. The fear is the stalk can break but many fibroids on a stalk have very short stalks. You're speaking to a GYN who is outdated in her philosophy. Removing healthy ovaries in my opinion is cruel. I have it legally stated mine may not be removed. A myo for fibroids on a stalk is a fairly easy laparoscopic surgery so you have choices. Drop that doc and get a modern one who sees the value in uterine preservation or go outside the speciality, to an IR, like I did.

I have a coworker who told me she had a hysterectomy 20 years ago and they went ahead and removed her ovaries when she thought she was only going to have the uterus removed. She was only 30 years old at the time and she was devastated. Perhaps I should also have something in writing.

Like the others have said I don't see the point in removing healthy ovaries. And do you have a family history of ovarian cancer? That just struck me as odd as the reasoning to go ahead and remove ovaries that are healthy .
I will be 45 at the end of the month and I had to have both ovaries removed. I was dealing with endometriosis and had several endometioma cysts on both ovaries. I had 1 ovary that only had 1 cysts on it. At my pre op apt I told the surgeon I'd like to try and save the one ovary with the 1 cyst. Hubby and I told the surgeon we'd leave it to his judgment on whether it could be saved. However, once my surgeon got in and found the one ovary we were hoping to save was attached to my bowel by endo and the blood inside the cyst was the cosistency of tar (he took several pics and reviewed with hubby after procedure ) he thought it best to remove it.
I trust my surgeon and his judgment. He was willing to attempt to save my ovary if it was salvageable. Keep looking. ..there are good surgeons out there. Mine was a gynecologic oncologist and not just a straight gyn.

Update - I went to see a different gynecologist this week. Unfortunately she also wanted me to have another uterine biopsy as she is a new doctor to me. I did have a lidocaine injection this time and it wasn't as bad as the first one I had.
I have decided on a hysteroscopic myomectomy. Far less recovery time than a hysterectomy. This doctor says someone will call me to schedule, but it may not be for a month to six weeks. I'm OK with that as I've never had surgery and probably need that time to gear myself up for it. I just want to stop bleeding - it's getting on for five months of endless period. I will be so glad if and when that stops. I have also quit my birth control pill. I'm bleeding anyway whether I take it or not and I can't help thinking they have helped the fibroids. This is just my opinion. The gynecologist says that hormones don't help the fibroids.
Thanks to all of you who post - I have been reading many posts here and they're very helpful.

Dianera... will your doctor be able to remove ALL of your fibroids hysteroscopically? From what I understand, that method is only used on pedunculated submucosal fibroids or very surface-oriented submucuosal fibroids (e.g., not able to use the technique on intramural).
If your doctor has to leave some of your fibroids behind, I suggest getting a referral to an interventional radiologist to see if you're a candidate for Uterine Artery Embolization/Uterine Fibroid Embolization. The procedure is fast and easy and shrinks the fibroids over time. Combining UAE/UFE with the hysteroscopic myomectomy could be a very effective course of treatment for you.
