Pelvis and Abdominal Swelling Six Months after TLH

I had a total laparoscopic hysterectomy done in September 2020 and I'm currently six months out. Around five/six weeks after my surgery, I noticed some pretty severe pelvic swelling which I just attributed to the healing process. I also had urinary irritation and genital and rectal burning but didn't have a UTI or an infection or bacteria of any kind. Fast forward to now, six months later, and I'm still swollen, with occasional urinary/genital irritation and random rectal burning. On top of that, around six weeks into recovery, I started to have abdominal swelling/fullness, which has only escalated. I look swollen 24/7. My abdomen feels heavy and full and it's hard to eat food without feeling like I'm going to burst. My diet has remained the exact same since before surgery (I'm a vegan) but I'm starting to eat significantly less than before because my symptoms are so physically uncomfortable and emotionally distressing. Looking down and seeing my stomach and pelvic area stuck out all the time is so disheartening and depressing. I hate the way my body looks now and I'm worried this is never going to go away. I've been to so many doctors and had so many tests done in the last six months and everything comes back normal. No doctor can give me any answers. I've seen posts on here about "swelly belly" and I'm wondering if I have that? But it is so borderline painful, I can't live like this for another second. Wondering if anyone else has experienced this and if it ever gets better? Is my body going to look like this forever? I'm terrified and depressed.

and welcome to HysterSisters, though I'm sorry to read of the issues which brought you here.

Swelly belly is the post-op swelling many of us experience in the 6-12 weeks after surgery due to the internal trauma - all those internal cuts to blood vessels, ligaments and cut-away organs. Typical swelly belly usually starts off minimal in the mornings and "gains momentum" as the day wears on. It usually abates in weeks, perhaps 2-3 months, and beyond the daily occurrence may come back and visit whenever we do more than our bodies are ready to handle. (5 months was the last time I dealt with it for a day or two.)

Your description does not sound like typical post-op swelly belly - 24/7, with urinary, genital and rectal issues, now appetite issues. My best guess (and that's all it is, a guess) would be something going on with your GI tract, but it doesn't explain it all. I'm wondering if you've got a resistant infection although it seems like blood, urine, fecal or vaginal swab tests would reveal something.

What doctors have you seen, what fields? Have you been to a gastroenterologist? Have you had any scans done (not just ultrasounds or x-rays)? CT scan with contrast?

I've seen posts here detailing swelly belly going on for years, that's why I thought maybe I had it. I've had a CT and MRI, both done in January. They didn't indicate much, but it was mentioned that there was soft tissue fullness behind my bladder which I just attributed to swelling but now I'm starting to wonder if it was something else. I've been to a gastro and had an endoscopy and they found nothing. My bowel movements are normal. I have had a persistent urinary infection, but only for about a month and a half now, it didn't start directly after surgery. My blood tests all come back normal. I'm completely lost and so is every doctor I see, seemingly.

Hmm, "soft tissue fullness" in January would've been 4 months post-op for you, and that would track a little more with swelly belly. Most of us are over it by 3 months, but there are some who have it lingering longer. I'd think that if it's years, it's some kind of chronic inflammation or digestive issues.

What have your activities been like? That's often a culprit for keeping inflammation going. It wouldn't explain everything, but some of it, and it may be you have more than one issue going on.

Have you tried cutting out most irritant foods and then adding them back slowly to see if you've developed a food intolerance? I've never had the need to do this, but a friend who nailed down her GI issues a gluten intolerance had done this for her family - allergies for one child, IBS for another, and her own gluten intolerance as well as something going on with her husband. I know there are guides out there, but you may want to discuss that with your doctor.

My final thought would be to consult with a specialist in chronic inflammation or infectious diseases. One of my Sister friends here had a stubborn yeast infection resistant to most treatments, and got referred to an infectious disease specialist who went deeper into her issues and finally found the cause.

I'm grasping at straws, but maybe something beyond "the usual" might make a little more sense.

I've been on and off working out, though recently off because I've been so depressed with the state of my body. But while I was consistently working out, no improvements. And the same with not working out.
I tried the LOW FODMAP diet for a couple weeks and nothing improved.
A couple specialists I've seen think it's pelvic floor dysfunction but I don't have any pelvic pain, just fullness in my pelvis and abdomen. And I haven't heard of abdominal distention being associated with pelvic floor dysfunction so I'm pretty confused.

Pelvis and Abdominal Swelling Six Months after TLH

I am 7 most post-op TLH and experiencing the same thing. My entire abdomen (rib cage to pelvis) remains swollen. I’ve had multiple surgeries prior my TLH, this was the first Robotic, and never encountered issues like this. It’s depressing to say the least. It’s been debilitating. I’ve done a lot of research and the closest diagnosis I can find is nerve entrapment syndrome. So sorry you're dealing with this too. Sending you much love and positive vibes in your healing.

Have you seen any doctors about it and asked their opinion? Or have you found anything that helps? I'm so sorry you're experiencing this too, it truly has sent my self esteem plummeting to its lowest level. I just want it to end. Looking up abdominal nerve entrapment, I also saw something called abdominal wall paresis and I'm wondering if it could be that? When did you start to get the swelling and has it gotten progressively worse? Because mine has.
