No BM yet 4 days after hysterectomy

I had a full abdominal hysterectomy on Tuesday, November 17, 2020. Ironically, that was the 49th birthday of my only child's father. The reason for surgery was severe anemia due to fibroids, one of which was the size of a football (this information was learned post surgery).

I was released from the hospital after just 1 day. I did have very loose bowel movements while still in the hospital but nothing since I've been home. I drank a very small amount of prune juice lost night hoping that would give me an urge to go. That didn't help so I had about 6oz this morning. After no obvious response after 6 hours, I drank another 6oz of prune juice. It's been 2 hours and still nothing. It has now been over 68 hours since my last bowel movement. I have Dulcolax but was delaying taking it because I don't want to have the runs.

I don't have any pain but I've completed my percocet 2 hours ago so we'll see if that changes.

Our bowels tend to be wonky after surgery - anesthesia, a side effect of the meds we're on, any bowel prep we did which can empty us out, and the trauma of surgery all can contribute. It can take anywhere from 3-7 days to make the first BM. Prune juice doesn't work like a laxative, so don't expect results at X-hours after taking it like you might with a laxative. It works in concert with other foods which add bulk or fluids. I'd have 1-2 small glasses during the day, and the next day I'd have action.

A diet with adequate fiber and magnesium aids in digestion and getting things moving: dark leafy greens, whole grains, fiber cereal, nuts, seeds, fruits (fresh, dried or frozen in smoothies), prune juice, warm liquids like tea, and enough water-water-water to swamp a canoe. Also, when you sit on the throne, prop your feet up. We're built to squat, and having your knees above your hips aligns the colon for emptying. Walking helps too - it doesn't have to be far or long, but the idea is to get up from the easy chair.

I'd stay away from the Dulcolax - that's a stimulant laxative which works by increasing contractions of the bowels. That's why they're, uh, explosive. After surgery, that can feel particularly awful, so avoid the bisacodyl found in Dulcolax and senna-based laxatives, both of which are stimulants. Instead, try Colace or another stool softener, or Miralax which draws water to the bowels.

My first bowel movement was on day 5. I second the walking, which has always been my top tip to get things moving. Warming up your prune juice can boost its effects; other top picks for a warm drink include peppermint tea, and a glass of warm water with the juice of half a lemon. The sugar alcohols that give prune juice its laxative powers can also be found in apple or pear juice, dried apricots, and sugarless gum. Dark chocolate is another excellent source of magnesium, and adding two daily squares to my usual high-fiber diet has helped to keep me regular. Another top tip to promote regularity is to squat on the throne, as Cussette mentioned, after every meal; this stimulates the gastro-colic reflex, in which the stomach tells the brain that it's full, then the brain tells the colon to empty its contents, to make room for the new stuff coming in. Coffee and chocolate always do it for me...and IMO taste far better than prune juice. A nice venti mocha latte is what you need! (also walking and lots of water as others said).

Do you have a catheter in? I've known some who just couldn't have a movement until the catheter was removed. No logical reason, I think they were just uncomfortable and it couldn't happen.
