What is the difference between vaginal estrogen and estrogen patches?

My gyno put me on Imvexxy 10mcg about 2 yrs ago but it does not help hot flashes at all.I had a hysterectomy and cervix and one ovary removed 5 yrs ago. I’m 57.
How is the patch different?

How is the patch different?
I am not a dr but have been using dottie .1 for 3 years. The way my dr explained it was that the vaginal creme stayed in the vagina stopping vaginal atrophy and the patch circulates through out the body. I rely on the patch to ward of arthritic pains and general wellbeing.

Again, not being a dr but my thoughts are yes. If I forget to switch out my patch on time my joints will let me know within 2 days. I did the research and for myself decided that the pros vastly outweighed the cons of the patch. I feel better (I look better) and I know that running out of estrogen really ages a body quickly. I'm not ready to go there yet. I have noticed that a general dr wants you off fairly quickly while an op/gyn dr doesnt usually push it. I will absolutely dr shop until I find one that doesnt plan on gatekeeping the estrogen lol
