Decision making process, intro and history

Hi,
I am awaiting an appointment with a gyn onc/surgeon. I requested a referral from my gyn, as I am trying to decide how to address my ongoing hemmoraging and simple hyperplasia without atypia.

My history: never pregnant, although went through fertility treatment (clomid) close to a year before adopting. 2 office hysteroscopies around 2003 and many ultrasounds found uterine polyp, which was removed during d &c/hysteroscopy/lap which also uncovered two dots of endometriosis on colon, which were cauterized. Continued to have short light periods with excruciating cramps like clockwork.

Fast forward to 10/2011.Got my period on the correct day but had no cramps. Optimistically hoped (with years of insomnia, increased migraines and other signs of perimenopause) that perhaps my period was going away. This was when the heavy bleeding began, along with increasingly longer periods. When I got to the point of a heavy period that wouldn't stop after three months, I got to a gyn who dxed simple hyperplasia without atypia and took magace for three months. Had a clean biopsy, went off the megace and 2 days later ended up being rushed to the hospital with labor pains and passed a three month sized clot when I stood to leave. Heavy bleeding started again after a year or so, back to a new dr. May 2014. Had a d &c June which revealed the simple hyperplasia is back.

Because the d&c has greatly reduced my bleeding, I have not been able to figure out what to do. I do not want the mirena (also have two 5 cm fibroids ) because I don't want to be at anyone's mercy to get it out if it's a nightmare, nevermind getting cramps again, bleeding or enlodging. Because of the clot, I won't do megace again. Sorry so long, but hope someone can advise. I'm 49, so future kids aren't on anyone's mind here, lol. Thanks for reading.
