Abdominal hysterectomy necessary for 9cm fibroid?
I had TAH after consult for UFE. Kept both ovaries. My dr's preferred method was TAH to be able ensure she could see everything since mine were large. Said she would have better vantage in the unlikely event something was knicked or punctured inadvertently. She did say laparoscopic was an option though if I wanted. She did a great job - no issues at all. I'm 8 months post op. Was home 8 weeks. Best wishes

I want you to stay hopeful and explore all your options with your doctor of course to have the least invasive surgery. I don't want to discourage you but just to be honest with my experience if it helps. All along my doctor (who is very reputable, had done my DNC and is part of the #1 hospital in my county) and I planned for a laparoscopic hysterectomy for my 13cm fibroid and the smaller one (plum size) which were external on the upper part of my uterus. I had planned for a 2 week recovery. To my utter shock, when I woke up, they told me they had to do a TAH after all. Doctor said "it was a difficult surgery." Though I completely understood the reasons why they couldn't dice up and remove the fibroids with the less invasive surgery, I was still so crushed they had to an unexpected TAH necessitating a longer recovery of course and new plans on everything (work, family, care). So just know you might have to be prepared or discuss with your doctor this possibility.

Hi there! Ditto with all the others. I think that I had a 17cm, 9cm, and a few 1-3 cm ones hanging inside and outside my uterus. Miraculously, my doc was able to get it laparoscopically with 5 keyhole incisions and a 2-3" bikini incision. Ultimately, I think it comes down to the doctor's experience and how comfortable they are with a particular procedure. Glad to hear that you are getting a second opinion and exploring your options.

It's great to hear all of your experiences. I would love to know how long it took you to recover from laparoscopic surgery, and Emarielee63 from abdominal hysterectomy.
I just saw a doc at UCSF fibroid center yesterday. She said she could do laparoscopic hysterectomy, but I didn't like how casual she was about the whole thing. She said she could do a myomectomy (if I understand correctly, that removes only the fibroids), although she had earlier said that she would recommend the cervix be removed (thus a hysterectomy) because of abnormal pap results recently and in the past. She also said we don't need to remove one ovary, while my ObGyn definitely recommends removing one ovary because of a 6 cm cyst that's way bigger than the ovary itself; she is concerned that it could twist and require emergency surgery.
I'm more confused than ever!

What did she say about embolization? Can't they remove the cyst and not the entire ovary?
It was a big turn off for me when they acted so casual about hysterectomy. That attitude inspired me to run. Scared me to death.

UCSF said that embolization would be $15,000 out of pocket because it is no longer in a trial and is not FDA approved.
My ObGyn said she preferred removing the entire ovary because attempting to remove just the cyst can lead to it bursting, and she is cautious about fluids escaping because, she said, an ovarian cyst cannot be biopsied until you have it out of your body, so if it turns out to be cancerous, and it bursts, it's very dangerous.
The other doctor--the one who was sort of super-casual about everything--didn't think the cyst or the ovary needed to be removed at all. To be honest I kind of got the feeling that the very young doctor at UCSF felt that my other doc was being old-fashioned, but I felt that the other doc errs on the side of caution because she has been practicing for many years and has seen women suffer through cancers that could have been avoided.

Are you sure she is not referencing focused ultrasound vs. UAE or UFE? Both are Radiology procedures. UFE is considered a viable option for fibroids and is recommended by the FDA where focused ultrasound is not. I would be confused too.

Okay--I think I misspoke. It was as you said ultrasound that isn't covered. Maybe embolization is. But embolization, if I understood correctly, would only shrink the fibroid about 30%, which would not solve any of my problems. And I don't like the idea of myomectomy because she said it's a 3-hour surgery, although a shorter recovery. Because I have a long history of fibroids, I would be worried they would come back and I'd be doing this all over again. I'm 46 and do not plan on having more children, so I have made peace with my uterus being removed.

My fibroid shrunk over 90% but I didn't expect that as I was told it would shrink no more than 50%. It is good if you are at peace.
