what exactly is fistula

Hi sisters,
I m sorry to sound so dumb or uninformed.
but what exactly is a fistula?
and what are the causes/ symptoms of one?
seems like there are so many posts about so many women getting them.

one reason iM so concerned is that I have sometime worse than other some leakage.
when I have the urge to urinate then I have to do so RIGHT THEN!
and when I wake up in the mornigns Im painfully about to go.
though no burning at the exit. only down one side like Im peeing glass as the urine passes down. sometimes I leak enough to wet my panties some and dont realize it until its already happened....
i cant hold my urine and when I have sex I sometimes leak!
I had prolapsed uterus before and my Dr. said that was the cause of it then, but Im still doing so. and some mornings I wake up alread have leaked quite a bit.

I have tried doing the Kegel exercises but it doesnt seem to help and it is as if those muscles are too weak or I just cant seem to do them..... I cannot hold my pee !

what do you suppose the problem might be?
what other things might cause these symptoms?

Hi Tina! I'm going to leave the explanation of what a fistula is to one of the ladies who have one... they are far better able to explain it than I am.

I wanted to address your question about what else could be causing those symptoms. It sounds like it could also be symptoms of vaginal dryness/atrophy and urethral atrophy. If there is not enough estrogen in the vaginal tissues, eventually the urethra will start to atrophy (wither) and it can cause urgency, incontinence and pain during urination. Doing kegels can help, if there is enough estrogen and testosterone in the tissues to keep the muscles there strong.

I see you are on an estradiol patch, and since you had endo your DR would probably not want to increase your dosage... but there are vaginal estrogen products out there that can help restore the health of the vagina and urethra without appreciably affecting your body's overall estrogen level. Hi Tina,

A fistula is actually a hole in your bladder, ureters, bowels etc. My fistula is a vesico vaginal fistula which is a hole in my bladder going into my vagina. It can either be a tiny pin prick or in my case as large as a quarter. A lot of fistulas are caused by radiation treatments. When you have a fistula you may experience constant leaking or only when your bladder fills up & spills out the fistula. They are usually accompanied by bladder spasms. Most of the cases on this site are due to surgeon nicking the bladder during our hysters. The only way to be sure if yours is a fistula is to have an IVP, cystograms, cystoscopies plus I have too many other tests to even remember done under the guidance of a urologist.
