Help! Gapping vagina after Perineorrhaphy and many repairs

Dear Sisters, This is my first post. I had a TAH/BSO five years ago, then about 8 months ago I was diagnosed with a rectocele and had it repaired 7-8-03. It wasn't bothering me all the time but I decided to get it repaired so it wouldn't prolapse further and bring all else down too.
Uro/gyn surgeon said that surgery for rectocele, cystocele,
enterocele, uterosacral colpopexy and perineorrhaphy would only be about 50% as bad as a hysterectomy...and I believed him!! This surgery has just about killed me and I am so down that I don't know what to do. I have never been one to get depressed but I want to cry everyday. My vagina has been shortened and my regular gyn said(after seeing the repairs) that if I have any more problems my vagina is in serious trouble. The repairs to the rectum do not make me feel any better, peeing is more difficult,
vagina scarred and hardened, and when I hold a mirror to look at the vagina, it is GAPPING OPEN! I can't believe I went through so much hell with this surgery and then have the vaginal opening larger and ALL systems worse than before. I do kegles and have tried all sorts of hormones. I feel as if I have entered the nightmare zone, I am so much worse off than before.
Can ANYONE help shed some light on my problems. I have a sinking feeling that my situation is not hopeful.
Thank you for letting me vent I have been reading your posts for a long time and they have really helped me through some dark times.

I am sorry you are going through such a hard time but where is your doctor? why don't you talk to him about all this?

Thank you for your reply. I have an appt. to see uro/gyn in 2 wks.
I saw him about 3 months ago and told him I thought my vaginal opening was larger than before surgery and he abruptly told me "no it isn't".

So I went home hoping that I was somehow wrong and that it would tighten up more as it healed. Unfortunately that has not happened and it is definitley larger than before the "tightening" surgery.

By the end of the day it is gapping open and you can acutually see up inside the vagina a bit (Dr. may not need to use a speculum, HA!).

My question is: Is it normal for a vaginal opening to be open?
Mine never was before. I would like to have a little more info before I talk with the Dr.

Oh ((((Jandie))))) I'm so sorry this last surgery has been so hard on you

I had a stage 3 rectocele repaired at the same time as my TVH. In my case, the repairs have resulted in a much tighter vagina than it was before. My hyst with repairs were almost 3 years ago and, despite having some issues with incontinence, the repairs are holding.

I understand from your post that you've consulted with your gyn since the surgery, but not with the surgeon who performed the surgery. If it is at all possible, I would go back to that surgeon and ask a few questions as to why you are still having problems and why your vagina is gapping. From there, you might want to consult another specialist in order to try and solve the problems.

You might also want to enquire about the feasibility of biofeedback pelvic therapy. It's basically kegels that are enhanced through a device that is inserted in your vagina and linked to a computer. I've been doing this since this summer and, providing I go for my therapy sessions , the results have been amazingly positive. My therapy is covered by my group insurance policy, providing it's prescribed by a doctor.

Thank you so much for your "soothing" reply. I really appreciate all the info and encouragement.

I have an appt. in 2 weeks with the surgeon so maybe he will be able to answer some questions. I dread seeing him because he has a very cavalier attitude and is the only uro/gyno that I know of in this area.

How does the biofeedback worK?

Hi (((Jandie)))) Hopefully, your uro-gyn will have a less cavalier attitude than in the past.... if not, how about consulting a "regular" uro who might be more sympathetic?

The biofeedback works through sending "signals" to the vaginal walls. This makes the muscles tense and release, the way kegels should work. There's no pain, at least for me, but there is a tingling sensation so that you know that it's working. It's all linked to a computer where the therapist can both control and monitor your progress. I know that there is undoubtedbly a more scientific way of describing the whole process, but, in a nutshell, that's how I saw it working.

Does the machine create all the tension or do you help with your own contractions too?

Also, do you think the vaginal opening is normally closed in a standing position?

Dear Jandie,

I am sorry you are suffering so. I have not had the same trouble as you, however, I have sent myself off to "distant parts" for second medical opinions and have had alot of success going that route -- at least answers that feel complete and round out my knowledge so I feel more empowered. It sounds like you might be at a point where considering something like this, though time consuming and maybe expensive, might really be a good thing for your feelings of hopelessness and control over the situation.

I too have done biofeedback (not for pelvic floor work, but for relaxation in the midst of great physical pain), and it truly was amazing how my feelings of pain could be controlled, as well some physical measurements (hand temp, etc.). I sure did come away feeling more hopeful, and always more relaxed and in less physical pain.

Jandie, with the biofeedback, the impulses are both from the machine and helped by the work of your own muscles. The biofeedback helps you, and the therapist, evaluate how you're doing see you can "see" how effective your muscle contractions and relaxations are.

As I mentionned before, in my case the perineal area has been a lot tighter since my hyst with repairs, so the vaginal opening tends to be closed. I would think that this is a question you need to ask your doctor or surgeon Sorry I can't be of more help.

I'm sorry to hear all you've endured How frustrated & disheartened you must feel after undergoing such an extensive amount of repairs & only to discover this

I have no personal experience w/repairs all tho one might be in my future <I am almost 4 yrs Post-TAH & was dx'd w/ a Vaginal Vault Prolapse> I did look up some info for you & came upon a few sites I to thot might be of some help to you:
