vaginal spotting after menopause

Hi, I just received a phone call from a older friend of mine. She is 68 years old and told me that she is having some vaginal spotting. I am so concerned about her. She has been divorced for years and is all alone. I am worried that this may be uterine cancer. She is having an intravaginal ultrasound today. She doesn't see a specialist and her GP is the one sending her for this ultrasound. Can anyone send some opinions to why a woman would have vaginal spotting long after menopause? I would appreciate any ideas or thoughts. Thanks so much!

Hello, Annie,

What a nice friend you are! I'm providing you with a link to the Merck Manual which explains that post-menopausal vaginal bleeding is due to endometrial cancer or atypical endometrial hyperplasia in about 20-30% of the cases.

Merck states that the bleeding may also be caused by estrogen or progesterone use or by genital atrophy secondary to low estrogen levels. Here's the link:

Before my diagnosis of endometrial cancer in April 1999, I had a transabdominal ultrasound which found a lesion which the radiologist concluded was a "probable fibroid." However, my gynecologist did not think that conclusion was correct and did an office endometrial biopsy which diagnosed my cancer. I have heard of other women who have had transvaginal ultrasounds that were normal, despite having endometrial cancer that was subsequently found on endometrial biopsy or D&C.

If I were your friend, I would want to consult with a gynecologic oncologist to make certain that nothing is missed in her diagnostic workup to rule out endometrial cancer.

Hi, Thank you so much for the information and the Internet link. I am concerned that she is not seeing a specialist. After reading the information, I will carefully suggest that she seeks an opinion of a specialist in this field. I also appreciate you sharing your experience. It is so nice of you to take the time to answer my concern!
