Bedroom problems

Hello, I'm 35 yrs old. I had a hysterectomy when I was 28. My last ovary was taken in 2002. I've had my ups and down with hormone therapy in the past but have finally found a good working system. I take Estrace 1mg 1/2 tab 3-4 times a day. Depending on the amount of stress or physical activity going on. I also use a Vivelle dot .1mg and take prometrium 200mg twice a week. I take bioidential testosterone once a week. This has worked out fine for me for since Dec. About 6 weeks ago my doctor put me on Paxil 10mg for stress and anxiety from an unrelated incident (I was in hit by a semi and it just kind of freaked me out, no one was hurt though). I've done great on the Paxil too. It hasn't interfered with anything. Last week though I began having problems with orgasm. It is nearly impossible. I know Paxil can cause problems but why now? It hasn't diminished my sex drive. One other thing that has been an ongoing problem is vag dryness. Despite all the estrogen and Vagifem which I use 3 times a week. Could the long term dryness be a factor in the other problem? I would just perfer to jump right on this and not let it linger. I'm just not sure which direction to look. Any suggestions would be greatly appreciated. Thank you.

Hi

Unfortunately a side effect of Paxil can be inability to reach orgasm. I took Paxil for 3 years following my LAVH/BSO and while I felt desire, it was very difficult to achieve orgasm, especially during the first few months of taking it. However, that side effect did eventually subside.... along with eventually finding my own hormone replacement "recipe" (which does include compounded testosterone).

We are all different. however, when it comes to how an SSRI will affect sexuality in the long term. It's up to you to weigh out the benefit of Paxil and how it may be helping with your own anxiety and/or depression. Hopefully, the side effects will diminish for you as they did for me. I'd say, give it a bit more time.

Regarding vaginal dryness...it surely can affect response as well. I have not tried Vagifem, but have had good results with Premarin vaginal cream. It's my understanding that Premarin vaginal cream is the most "potent" when it comes to treating vaginal dryness concerns...altho it is a bit messier.

I'm considering uping the times a week I take progest to see if that might help too. Its worth a try.

(((butterfly))) It's very unlikely that increasing your progesterone is going to help with your ability to experience orgasm... in fact, it's more likely that it will work against you. As (((Shelley))) said, Paxil is known to cause problems with orgasm. Have you and your DR discussed any other alternatives that might not have that side effect?
