keep cervix?

Hello all! I'm new here...scheduled for a davinci assist hysterectomy on 2/26. I have large fibroids that have been causing some problems. My doc has described the size of my uterus/fibroids as "about like a 4 month pregnancy" one of them about 10cm and none were present at my last exam just a year ago.

I am looking forward to get my quaility of life back but am having some questions in the back of my mind. My first OBGYN was quite insistent that I have my cervix removed with my uterus. I have a history of abnormal paps that resulted in a LEEP procedure in Oct. 2004...normal paps since then. However, given that history he said he would take the cervix out at the same time. This doctor has been fantastic through out my history with him but is known as quite conservative.

In contrast, the doc I went to for a second opinion/inquiry about the davinci procedure suggested I leave the cervix in, citing recent research about improved sexual satisfation and continence post hysterectomy. He also said that if I needed to go ahead and remove the cervix later that it isn't really a big deal...just another procedure later.

Any opinions on this either way? I thought I had it settled in my mind that I would keep the cervix as long as the pap I just got on the 19th came back clear but now I'm not so sure.

My surgeon agreed to leave my cervix unless she found signs of cancer in which case she was taking it out. It's gone and I'm OK with that.

You asked for opinions so I think if I were in your place, I'd rather not go through another procedure later.

It is true that most women are cured from their abnormal cells after a LEEP (80-90% are cured). After my first cone biopsy, I had normal PAP's for 10 years and then one came back abnormal. Then another and another. I had several more procedures and finally the hysterectomy. Obviously, I fell into that 10-20% that are NOT cured with a LEEP.

If you require a trachelectomy later (removal of cervix), the recovery period can be anywhere from 2-6 weeks--I've read varying reports on that.

My cervix is gone. I don't miss it. I don't think mine ever contributed much to my sex life. Not having any continence issues--I'm 6 years postop.

It is my understanding that it is still recommended practice to remove the cervix with a hysterectomy if there has been a history of abnormal PAP's. I tend to be on the conservative side myself, given my history. And even with my cervix removed, I have had several abnormal PAP's over the years. Do what you will regret the least (one of our favorite pieces of advice). If you have an abnormal PAP down the road, will you be mad that you didn't have your cervix removed, or will you be OK with that? Different women will answer that differently, and there is no right or wrong choice.
