Lower abd pain, post op 6 weeks HELP! PLEASE

Hi everyone. I posted before I had my surgery on 11/29/07. I was nervous and unsure I wanted to go through with the surgery. You were all so great. I got lots of good info and I had my surgery on 11/29/07. Vaginal kept my ovaries. All went very well. Home in 2 days, off pain meds in 4. Only issue is a nagging lower left quadrant abdominal pain. I asked the dr. on my 3 week visit what this was about. Its bad when I stand up, cough, sneeze etc. She said take some advil it will go away. Well I am now going back to work on Thursday (2 days) and its getting much worse. I was able to get in to see the Nurse Pract tomorrow morning, as the dr. isn't available until NEXT MONDAY! She didn't want to see me at all after my 3 week visit. But now this pain. Its very localized. I can actually put my finger right on the spot. I am wondering if its a lesion or a hernia caused by the surgery? What do you think? <oops! no private email addresses, please - use PM/email buttons> THANK YOU SO MUCH

Hi Are you having any fever with those lower left abdominal pains? Do you notice any association with bowel movements? These would be important things to share tomorrow at your appointment.

If you don't have those things, it's possible you may have an abscess or small hematoma in that area. Hernias usually occur along incisions in women with a TAH, not in vag hyst ladies. Perhaps they could do an ultrasound tomorrow to see if they can see a problem in the area that is hurting?

I haven't taken my temp but I don't feel like I have one. I will go take it right away. The pain started immediately after surgery which is why I think the Dr. wasn't worried. It did, at first, seem to be associated with bowel movements. If I was a little constipated it seemed to be worse, but not now. Now its 24/7 and getting worse. I would say its like a cross between a "stitch", a pain and a burning pinch. Last night I thought I would end up in the ER. Today its not as bad but I have only been up for an hour. And I can feel it totally. I see the dr. in 2 hours, well the nurse pract that is. I will ask about an ultra sound. I didn't even think about that. Since the pain started right away do you think it could be an abscess? Thanks again!

Since it started right away, maybe not an abscess but it could be a hematoma (essentially a big internal bruise; tends to form a pocket of pooled blood and can put pressure on surrounding tissues, causing pain). I would ask to see the actual DR, not just the NP, while you're there. Let us know what you find out.

Dr. wasn't available until NEXT WEEK! The soonest I could get in with her was next Monday. I couldn't wait that long. This pain is really getting worse. But the NP did a pelvic and took my temp. Temp is normal. She didn't feel anything inside and said I am healing nicely But they did take a urine test and I did show positive for bacteria. So... maybe its just an odd uti. They gave me antibiotics that I already stated and ordered an Ultrasound. The NP said she wasn't comfortable doing an ultrasound herself (only to look for babies, not other things). I get the ultrasound on Friday. I figured give the antibiotics two days to take hold and if I feel better then cancel the ultrasound. But if not go for it then. Otherwise they would order it as a rush and get me into the hospital today. But since my dip test did show something, and my urine was a pretty shade of orange... no blood in the urine though... it could just be a localized UTI Right? I don't have pain when I urinate or anything else... so hopefully thats it. I've had UTIS before this is not like any of them. This is so localized... your hematoma makes a lot of sense to me! I might just keep the ultasound appt no matter what! THANK YOU. The more I type the more I think you might be right AND I have a UTI. That makes total sense. I wonder if an ultra sound would show a hematoma? What do you think?

It should... I've never had a pelvic hematoma, but I did have one in my breast after a lumpectomy and it was diagnosed via ultrasound. They used ultrasound to track its progress as they waited for it to soften and start to drain. That was a year and a half ago; it's still there but not a hematoma anymore, now it's a gradually shrinking seroma (filled with clear fluid).

The orange shade of your blood could be due to the dyes in meds or vitamins... I get that after taking certain supplements (I suggest that only because you already said they didn't see any blood in it).

Not to scare you, but it could also be a nicked ureter, and you want to know about that as soon as possible. I believe the ultrasound could also see that, although not in great detail.

I would be tempted to keep the ultrasound appointment even if you're a little better by Friday, just for peace of mind.

VERY GOOD. I will keep the Ultra Sound appt for sure. Do you think I would see any urination problems with a nicked ureter? Or other symptoms? Or what ARE the symptoms? I just spoke to another dr. (Had to take hubby to dr as he threw out his back) and asked her. She also said yes a hematoma would show up. She said that "maybe" a UTI would cause the pain I am describing, but... she kinda hemed and hawed at that one. The Hematoma sounds so much like it makes sense. If I hadn't asked the NP for an ultrasound referral she wouldn't have even suggested it. Are we having fun yet?

At least I feel like I have some ideas now as to what this pain is. I will take the antibiotics and keep the ultra sound appt. Then at least I have both basis covered and I won't be as worried. Except for the nicked issue Oh and on that line again.... My surgery was 6 weeks ago... do my symptoms make sense for that?

Hi Marcie!

I'm sorry you are having this problem.

Your surgery was six weeks ago....there is still a lot of healing going on inside, and that could be causing pain. Hopefully, there is nothing else major going on !

I'm glad you are keeping your ultrasound appointment as they will really be able to see what's happening inside there. Keep up with the antibiotics and hopefully, you will have some answers tomorrow after the U/S.

The pain is not at all as bad today. So I think maybe its a combo of both UTI and maybe the hematoma? But either way I WILL keep the US appt, thats to YOU guys!! I can't thank HS enough for the help and being there!! I love this site.

I went for the ultrasound on Friday. I could tell right off she found something as she went RIGHT to the pain. I knew they are not supposed to tell you anything but I did ask her if she saw something. She said YES. When I was done she said it looks like I have an ovarian cyst. Now my question.... My dr, during the surgery, looked at both ovaries. She said they were fine and thats why I asked her to leave them. But within days of the surgery I started with this sharp localized pain which appears to be where the cyst is. Could a cyst have developed in such a short amount of time? I would say no less than 4 days? Does that make sense? So now what??? Another surgery to remove the cyst? The antibiotics don't seem to have reduced the pain, maybe a little, but not much. Its not 24/7 now, mostly just when I cough and at the end of they when I am relaxing. Without this I would have said my surgery was a breeze... THANK YOU
