Meeting w/ Doc to discuss hysterectomy

Hi, I am new to Hystersisters and would appreciate any advice.

I go to talk to my specialist (reproductive endocrinologist) this week about a complete (removal of uterus & ovaries) abdominal hysterectomy. I have the following conditions that I have been battling for (14) years, I am 33 1/2 years old. PCOS, endo, migraines (hormonal) high lipids (pcos) DUB, IC, chronic pelvic pan (lower left) and lower back pain (lower left) and prolapsed uterus. After 3 D & C in the past 6 months there are 2 procedures that I am considering. I also had 6 months of Lupron shots in 2003.

#1 endo ablation. However, this will not do anything for other conditions, just my chronic bleeding, but due to my lining growing back so fast, the docs are afraid this may be a temporay situation.

#2 complete (abdominal) hysterectomy (removal of ovaries and uterus) According to my OB/GYN they would need to remove my ovaries because of the endo, but are concerned about HRT since I have high lipids. I would also be getting a bladder lift at the same time.

I don’t know which procedure would be best for me. My family doc. thinks I may be trading one condition (pain and bleeding) for another (possible heart condition) Any advice?

Hi Tucker,

Sorry you have so much 'going on in there' and that you are in a lot of pain. Sounds like you are really suffering with all that stuff, but that it may be worth it to be rid of the pain and get your bladder lift. I'm trying to hold off on the hyst for now, I'm 36, no kids but want them. I don't know what to tell you, only you know in your heart and mind what is best for you.
If you are really suffering and feel like the quality of your life has gone way down, it may be worth it. Can your doctor monitor you more closely for heart problems after surgery?

Thank you for your reply. I will be meeting with my doctor tomorrow to discuss all of my options. This will be my first meeting with this Dr. (he did my emergency D & C in Feb). He is a partner with the Dr. I see all the time. This Dr. specializes in pelvic floor/rectal dysfunction due to prolapse, pelvic surgery for treatment of urinary bladder incontinence and diagnosis and treatment for osteoporosis.

After speaking to him I feel I will be able to make a better decision. I will be asking about an endometrial ablation w/ pessary (for uterine prolapse) or if there is some kind of lift he can do with the uterus and bladder. My other option is hysterectomy. #1 Leaving the overies and dealing with the endo. or #2 Removing the overies and my risk of cardiovascular disease.

I hope your appointment tomorrow goes well!

Would you be able to consult with an endometriosis specialist? I had my hysterectomy for endometriosis but retained both of my ovaries as that was the best option for me. Removing the uterus and/or ovaries does not cure endometriosis (or PCOS for that matter). The key for endometriosis is to have as much endometriosis as possible removed by a skilled surgeon who treats endometriosis. Not all surgeons are skilled at recognizing the various appearances of endometrosis nor can they remove it from all the areas where it likes to implant such as the bladder and bowels.

I would suggest talking to each doctor and weigh all of the pros and cons specifically for you. You may also want to seek the opinion of someone who does not work with your current doctor just to get a completely different viewpoint and see if it aligns with the advice you have already receive and what you hear from the reproductive endocrinologist.
