Does the cervix need to go?

Greetings all. I'm so glad to be here!

This is my first non-introduction post. On March 10th I was diagnosed as having endometrial adenocarcinoma, grade 1. The gynecologist (who also specializes in pelvic reconstructive surgery and comes highly recommended) has me set for LAVH surgery on April 9th. As my cancer is hormonally linked, he wants to remove my uterus, tubes, ovaries and cervix. Does anyone know if the cervix removal is a "wise necessity" in this case or just the ovaries and tubes and rest of the uterus? I'm wondering if I should try to keep it or not. I'm 58 years old and this will be my first surgery. So much to learn in a short time!

Also, reading a lot on this website, I see repeated advice about seeing a Gynecological Oncologist. The one I called (that an aquaintance had mentioned) said it was protocol to get a "referral" from my doctor. Since my doc wants to do the surgery, this is awkward. Any suggestions? THANKS! Also, should I postpone the surgery or is there time to see another doc before then?

Ask for the referral. My GYN wanted to do it so balked at the request until postop when I found out seeing GYN onc was standard of care. Then I needed another surgery to to what should have already been done, he gave me the referral then. So ask and asking to see a GYN onc IS standard. Cervix needs to go, for staging. Stage 2 is spread to the cervix, they need to check it. Its all new and a shock at first but it gets better. Reading up on grade and staging helps. Good luck and see that onc, truly.

If you don't have a Gyn-onc do the surgery than minimally an oncologist needs to be present. My original gyn was going to do that until I found a gyn-onc that could do it via the daVinci process instead of abdominally.

I was ready to postpone my surgery if the gyn-onc couldn't get me in for a consult before my original surgery. So I would say go with the 2nd consult.

I definitely go with the gyn/onc, otherwise you may be facing a second surgery later to remove lymph nodes in order to look for spread of the cancer (staging). The removal of the cervix is part of the staging process as well.

Don't feel bad if you tell your gyn that you want to look at other options. They should be used to hearing this. It's your body and your decision...and your future, not theirs. I talked to 3 drs before finding one that felt would do the best job.

Until reading posts on this Web site, it had not occurred to me that some people had their cervix removed and others did not. When I asked me doctor, he told me that when cancer is the primary reason behind a hysterectomy the cervix should be also be removed. Again, this is just one opinion.

Golunda, I too would seek the services of a gyn/onc.
My regular gyn opted out of my surgery and sent me to a wonderful gyn/onc. He knew I would need more surgery than he was comfortable or qualified to complete and did not want me to have a second surgery.
You should ask your gyn for a referral. Better to bear the embarrassment or hurt feeling (hopefully not tho) rather than face another surgery that could have been avoided.

I want to thank each of you for your kind and helpful responses! Your steady urgings for me to see a gyn/onc bolstered my courage. After some research of gyn/onc's and hospitals, as of this morning, I now have an appointment for Wednesday, March 31st. I'll take copies of my initial reports with me and we'll see where things go from there. Thank you, thank you. You've been just what I needed.

And thanks for the cervix info too. Now I'll be ready now for what the gyn/onc says too. Thanks!

Hey - way to go! You probably can still get an LAVH from the gyn/onc - I did. I guess it's all what they are comfortable with and what your tests show already.

That really depends on you! I did not think my cervix should go either, but when they did my TAH two weeks ago, they found lots of long-term serious infection, so they took the cervix. I had no idea that anything like this was there.

Listen to and trust your docs, as we are all different. I was so surprised to learn that I had something unexpected. I guess I shouldn't have been, but when you're 39, sometimes you still think you are invincible!

In the end, my doc knew what he was doing. Hopefully you feel comfortable enough to know that, too.

good for you golunda...wise choice to seek a gyn/onc for treatment. in my case, ovca, my cervix was not removed. i think it depends on what kind of cancer you have, whether it is likely to spread through the cervix or not. it's been so long for me now, i don't remember where i heard it, that ovca was more apt to spread somewhere other than the cervix. wishing you all the best at your appt. and please let us know how it goes. take care.
