TLH vs. LAVH

What is the difference between TLH and LAVH? Both use laparoscopy and remove the uterus through the vagina, so what is the main difference?

The TLH isn't done vaginally--the uterus and other parts are removed through the laparoscopic incisions. See the Hysterectomy Options link at the top of the page, and click on the link for reading more about Laparoscopic Hysterectomy.

My dr. said I will be having a total laparoscopic hysterectomy, but since I am also having the cervix removed, he can remove it all through the vagina. If he had to remove it through the laparoscopic incisions, he said he would have to use a morcellator to cut it into strips. Maybe that's just the terms that my dr. uses?

The terms are confusing to me too. I had a TLH and my uterus was removed through the vagina. I had no idea that would be the case, and the uterus wasn't especially huge, but I guess my surgeon felt it was best to remove it this way and I think that is fairly common. Good luck with your TLH and have a smooth recovery.

It is confusing. LAVH is a laparoscopic assisted vaginal hyster. The uterus is removed through the vagina and lap tools are used for visualization and to assist.

TLH is a total laparoscopic hyster. Everything is removed through the lap ports using a morcellator to make things small enough.

Both hysters require a vaginal cuff when the cervix is removed. Your doc has the option of removing everything vaginally. If he does the TLH will become a LAVH.
