Heads Up - Disability Insurance Changes

Not sure if it's okay to post here but wanted to pass along this information. It's for any of you ladies with upcoming surgeries and filing for disability benefits.

On March 1, 2013 they changed the forms. If you have a black and white form, they won't accept it. My forms were given to me by my employer. While my surgery was on February 21st, they sent them back to me! Had to redo on new white and red forms, then resubmit. And then I actually drove them to and hand delivered them to Disability Office.

It will take a week before you see/hear anything. I won't see benefits until after I go back to work (3/25th). In finally speaking to a representative, they said it is much easier (and quicker) to apply online. This is also a new process but you just register, right down the id number it gives you. Give that number to doctor so they can go in and fill out their portion.

Just thought I'd pass along this information so that you ladies know what's happening.
